摘要
目的评价显微超声技术结合锥形束CT(CBCT)检查治疗老年钙化根管的临床效果,为钙化根管治疗提供新思路。方法选择我院40例共52个根管经传统根管治疗失败的老年患者,利用CBCT辅助检查确定手术入路,再应用显微超声技术寻找根管入口并治疗。结果 52个钙化根管经CBCT检测定位并行显微超声治疗,43个疏通,疏通成功率82.7%;9个根管因钙化范围广、近根尖及弯曲率高等,磨除3~4 mm后CBCT检查仍无法找到根管入口,治疗失败,仅予充填并随访。结论 CBCT检查结合显微超声技术是治疗老年钙化根管的有效方法,有助于提高治疗成功率。
Objective To evaluate clinical effect of cone beam CT (CBCT) combined with dental operating microscope and ultrasonic technique in treatment of elderly patients with calcified canals so as to develop a new calcified canals method. Methods A total of 40 elderly patients (52 root canals) who failed in traditional root canal treatment were examined by using CBCT auxiliary examination to find the operating pathway, and the dental operating microscope and ultrasonic instruments were used to find root canal pathway in treatment. Results Among 52 calcified root canals, 43 root canals were dredged with the help of CBCT auxiliary examination and using the dental operating microscope and ultrasonic instruments therapy, and the achievement ratio was 82.7% ; 9 root canals were failed to find pathway by using CBCT examination after the operating pathway was grinded about 3 -4 millimeters because of the large coverage of calcified materials, near root tip and the high rate of curving. The 9 calcified root canals were filled up after unsuccessful treatment. Conclusion The dental operating microscope and ultrasonic technique combined with CBCT are efficient in treatment of elderly patients with calcified canals.
