摘要
目的探讨六西格玛理论在降低新生儿监护病房(neonatal intensive care unit,NICU)非计划性拔管中的应用效果。方法选择2012年4月至2012年10月在我院NICU住院患儿185例为实验组,应用六西格玛理论对非计划性拔管的主要原因及其影响因素进行总结分析,同时制订并实施改善措施;选择2011年4月至2012年10月在我院NICU住院患儿161例为对照组,施行常规护理。比较两组非计划性拔管发生率的差别。结果运用六西格玛管理理论模式,分析得出NICU患儿非计划性拔管的主要原因,包括患儿自身原因、护理人员操作不规范及导管质量问题等,同时根据这些原因制定了相应改善措施并加以实施,使非计划性拔管率明显下降,实验组患儿非计划性拔管率由实施前的4.32%下降到1.24%,达到了预期的目标。结论应用六西格玛理论对NICU非计划性拔管进行风险防范管理能够有效地降低NICU患儿非计划性拔管发生率,从而提高护理质量,确保患儿顺利进行治疗,早日康复。
Objective To explore the effect of applying Six Sigma theory in reducing NICU (Neonatal Intensive Care Unit) unplanned extubation. Methods 161 pediatric patients from April 2011 to October 2012 were divided into control group and received routine nursing. 185 pediatric patients in our hospital's NICU from April 2012 to October 2012 were divided into experimental group. Unplanned extubation were analyzed with Six Sigma theory, and corresponding measures were conducted. Two groups' incidence of unplanned extubation was compared. Results With the application of Six Sigma theory, reasons of unplanned extubation were concluded, including pediatric patients' factors, nursing staffs operation and catheters' quality. Corresponding measures were conducted, and unplanned extubation rate of the experimental group dropped from 4.32% to 1.24%. Conclusions The application of Six Sigma theory can effectively reduce the risk of unplanned extubation in NICU, help to improve nursing quality and promote pediatric patients' rehabilitation.
