摘要 目的研究不同肠内营养(enteral nutrition,EN)液温度对大鼠胃缺血再灌注损伤(gastric ischemia-reperfusion injury,GI-RI)的影响及其可能的作用机制。方法将40只SD大鼠随机分为EN液温度10、24、32、40℃4组,夹闭腹腔动脉30min造成大鼠胃缺血,松开动脉夹后再灌注形成GI-RI,术后2h开始给予EN液。GI-RI 48h后10倍放大镜下测定胃黏膜损伤指数,光镜下行胃黏膜损伤病理评分,测定胃黏膜丙二醛(MDA)含量及超氧化物歧化酶(SOD)活力。结果 EN液10℃组胃黏膜损伤指数、病理评分、MDA含量均低于其他各组(P<0.05),SOD活力高于其他各组(P<0.05);EN液40℃组胃黏膜损伤指数、病理评分、MDA含量均高于其他各组(P<0.05),SOD活力低于其他各组(P<0.05)。EN液温度与胃黏膜损伤指数、病理评分、MDA含量呈正相关(均P<0.01),与SOD活力呈负相关(P<0.01)。结论降低EN液温度对大鼠因缺血再灌注造成的急性胃黏膜损伤具有保护作用,其机制可能与低温促进机体过氧化与抗氧化平衡有关。 Objective To explore the effects of enteral nutrition (EN) solution temperature on gastric ischemia-reperfusion injury(GI-RI) in rats and to discuss its possible mechanisms. Methods Forty Sprague- Dawley(SD) rats were randomly divided into 10℃, 24℃, 32℃ and 40℃ groups according to EN solution temperature. GI-RI was induced in rats by clamping the celiac artery for 30 rain and then reperfused when the celiac artery was released. EN was started at 2h after the surgery. The gastric mucosal injury index was counted grossly at 48h after the GI-RI. The pathological score of gastric mucosal injury was evaluated under a light microscope. Then the malonaldehyde (MDA) content and superoxide dismutase (SOD) activity of gastric mucosa were measured by colorimetric method. Results Compared with the other, groups, the values of gastric mucosal injury index,pathological score and MDA content in EN solution 10℃ group were decreased(P〈0.05) ,while the SOD activity was increased(P〈0.05). Meanwhile,the values of gas tric mucosal injury index,pathological score and MDA content in EN solution 40℃ group were higher than other groups(P〈0.05 ), but the SOD activity was lower than other groups (P 〈 0.05). Furthermore, EN solution temperature was positively correlated with gastric mucosal injury index, pathological score and MDA content(P〈0.01),and it was negatively correlated with SOD activity(P〈0.01). Conclusion The decrease in EN solution temperature exhibits a protective effect against acute gastric mueosal lesions induced by ischemia-reperfusion in rats,which is probably involved in promoting the balance of peroxidation and antioxidation by hypothermia.
