不同剂量右美托咪定用于高血压患者清醒气管插管的效果 被引量:22

摘要 目的观察不同剂量右美托咪定用于高血压患者行纤维支气管镜(FOB)清醒气管插管的效果。方法择期全麻手术患者100例,年龄20~60岁,高血压1或2级,随机分为五组,每组20例,插管前15min,分别静注生理盐水10ml(C组)、舒芬太尼0.1μg/kg(S组)或静脉泵入右美托咪定0.5、1.0、2.0μg/kg(D1、D2、D3组),所有患者插管前均行气道表麻。记录基础值、插管前即刻和插管后即刻的MAP、HR和BIS值,记录插管时间及一次插管成功率及插管时不良反应。结果气管插管前即刻D2、D3组MAP明显降低,HR明显减慢(P<0.05);气管插管后即刻C组、S组和D1组MAP明显升高,HR明显增快(P<0.05);插管前即刻除C组外BIS值均显著下降,插管后即刻D3组BIS值明显低于基础值(P<0.05)。C组一次插管成功率较低,D2、D3组气管插管时间明显缩短(P<0.05);S组和D3组呼吸抑制发生率较高(P<0.05),C组、S组和D1组高血压、心动过速、躁动的发生率较高,而D3组心动过缓发生率最高(P<0.05)。结论在完善的气道表麻基础上,静脉泵入右美托咪定1.0μg/kg可安全有效地应用于高血压患者FOB清醒气管插管。
