摘要
目的研究产前筛查降低先天缺陷儿出生率的价值。方法选择2010年2月-2012个3月深圳市罗湖区妇幼保健院8300例孕中期孕妇分别进行血清标志物甲胎蛋白(AFP)和游离绒毛膜促性腺激素(Free-βhCG)二联筛查与AFP、Free-βhCG及μE3三联筛查,所有孕妇均进行跟踪直至新生儿出生,以确定其在降低先天缺陷儿出生率的价值。结果在风险切割值1∶250共检出DS高风险713例,阳性率8.59%,确诊21-三体综合征7例,占高风险孕妇的0.98%;而三联法检测结果显示,DS高风险例数为321例,确诊21-三体综合征9例,占高风险孕妇的2.8%,两种方法检查阳率比较,差异有统计学意义(P<0.05)。共检出2NTD高风险孕妇62例,阳性率0.75%;62例均经超声检查,发现8例异常(占12.9%),其中2例死胎,3例无脑儿,2例脐膨出,1例腹裂肝外翻。所有确诊异常的孕妇均接受引产证实,符合率100%。结论孕中期产前筛查简单易行,必要时使用AFP、Free-βhCG及μE3三联筛查可提高准确率,具有重要的诊断价值。
Objective To study the prenatal screening reduce birth defects son the value of the birth rate. Methods Choice in February 2010-2012 March 8300 cases of pregnant women serum marker middle respectively a tire protein (AFP) and Free human gonadotropins (Free-β hCG) two league screening and AFP, Free-β hCG and μ E3 press screening, pregnant women are all follow up until birth to determine the lower birth defects son in the value of the birth rate. Results In the risk curing value pitted 0, a total of 713 patients at high risk detection DS, positive rate was 8.59%, 21-confirmed three body syndrome 7 cases, accounting for 0.98% of the pregnant women at high risk, and triple results show that the method to detect, DS high-risk number for 321 cases,diagnose 21-three pyramidal syndrome 9 cases,accounting for 2.8% of the pregnant women at high risk,two kinds of methods to check Yang rate compared with significant difference,there is statistical significance P〈0.05. Detection of 2 NTD 62 cases of high risk pregnant women, positive rate was 0.75%; 62 cases were by ultrasound examination, found abnormal 8 cases, accounting for 12.9%,2 cases of stillbirth,3 cases the,2 cases of umbilical surfaces and 1 case with liver abdominal crack out. All pregnant women are diagnosed abnormal accept induced labor confirmed that coincidence rate 100%. Conclusion The middle with simple prenatal screening and,when necessary, use AFP, Free-β hCG andμ E3 press screening can improve the accuracy, has the important diagnostic value.
