摘要 目的探讨CT与MRI两种检测手段在诊断早期股骨头坏死中的应用价值。方法选择我院2010年1月~2012年1月收治的56例(98髋)确诊为股骨头坏死的患者,对其进行l、2、3、4期CT及MRI影像诊断,探讨其诊断准确率及临床表现。结果检测1~2期敏感性CT为62.5%(15/24),MRI为87.5%(21/24),差异有统计学意义(χ2=4.00,P<0.05)。总体敏感性、单一囊变上两种检测比较差异无统计学意义(P>0.05)。骨小梁结构模糊CT检出率为8.3%(2/24),MRI检出率为70.8%(17/24),差异有统计学意义(χ2=11.73,P<0.05)。结论在检查股骨头坏死的临床诊断中,MRI在股骨头坏死早期诊断率高于CT,在早期的细微的骨小梁结构模糊的诊断率方面,MRI显著高于CT,MRI用来诊断早期股骨头坏死有一定的临床应用价值。 Objective To discuss the application value of CT and MRI in early diagnosis of osteonecrosis of the femoral head. Methods A total of 56 cases (98 hips) in our hospital from January 2010 to January 2012 ,who were confirmed as osteonecrosis of the femoral head,were diagnosed by 1,2,3,4 phase CT and MRI. The diagnostic accuracy rate and clinical manifestations of CT and MRI were discussed. Results 1-2 phase detection sensitivity of CT and MRI were 62.5%(15/24), 87.5%(21/24). There was statistically significant between two kinds of detection methods (X2= 4.00, P 〈 0.05). The differences of overall sensitivity and single cystic change were not statistically significant (P 〉 0.05). The trabecular structure fuzzy detection rate of CT and MRI were 8.3%(2/24), 70.8%(17/24). There was statisti- cally significant (X2= 11.73, P 〈 0.05). Conclusion In the clinical diagnosis of osteonecrosis of the femoral head, the diagnosis rate of MRI is higher than CT in early. The rate of MRI in the diagnosis of early small bone trabeeula fuzzy is significantly higher than that of CT. MRI has certain clinical value to be used for the diagnosis of early osteoneero- sis of the femoral head
