摘要
目的观察地佐辛复合丙泊酚用于无痛人流术的麻醉效果和安全性。方法选择我院门诊2012年3-10月收治的自愿要求终止妊娠的早孕妇女60例,均行无痛人流术。采用随机数字法分为两组:观察组应用地佐辛+丙泊酚麻醉;对照组应用芬太尼+丙泊酚麻醉。记录两组术中生命体征及术中镇痛效果、丙泊酚用量、麻醉恢复时间、术后疼痛、不良反应的发生情况。结果所有患者均顺利完成手术,无麻醉失败。注药后2min,2组患者收缩压(SBP)、舒张压(DBP)、心率与麻醉前相比较,差异均有统计学意义(P<0.05);观察组与对照组注药后2 min血氧饱和度(SpO2)比较,差异有统计学意义(P<0.05)。两组丙泊酚总用量和术中镇痛效果分级比较,差异均无统计学意义(P>0.05);两组术中呼吸抑制比较,差异有统计学意义(P<0.05)。两组唤醒时间、定向力恢复时间、术后恶心呕吐比较,差异均有统计学意义(P<0.05);两组苏醒后下腹痛VAS评分比较,差异无统计学意义(P>0.05)。结论地佐辛复合丙泊酚应用于无痛人流术麻醉安全可靠,镇痛效果满意,而且地佐辛复合丙泊酚麻醉术中呼吸抑制发生率、术后不良反应明显低于芬太尼复合丙泊酚麻醉,地佐辛复合丙泊酚麻醉恢复时间短于芬太尼复合丙泊酚麻醉恢复时间。
Objective To investigate the effect and the safety of dezocine combined with propofol on induced abortion of indolence.Methods 60 early pregnancy women scheduled for painless induced abortion were randomly divided into two groups(n=30):observed group undergoing the intravenous anesthesia by dezocine combined with propofol,and control group receiving fentanyl combined with propofol.Record two groups of intraoperative vital signs and intraoperative analgesic effect,propofol dosage,anesthesia recovery time,postoperative pain,the happening situation of adverse reactions.Results The rate of intraoperative respiratory inhibition in group D is significantly lower than the group F(P&lt;0.05),anesthesia recovery time in group D is significantly shorter than group F(P&lt;0.05),the incidence of postoperative nausea and vomiting of group D is obviously lower than group F(P&lt;0.05).The total dosage of propofol and the effect of intraoperative pain in group D and group F has no significant difference(P&gt;0.05),postoperative analgesia effect in group D and group F has no significant difference(P&gt;0.05).Conclusion Dezocine combined with propofol used in painless abortions is safety and reliability,analgesic effect is satisfactory.The incidence of intraoperative respiratory depression and postoperative adverse reaction in dezocine combined with propofol anesthesia is significantly lower than the fentanyl combined with propofol anesthesia,anesthesia recovery time in dezocine combined with propofol anesthesia is shorter than fentanyl combined with propofol anesthesia.
