摘要
目的分析T-SPOT.TB检测结核感染的敏感度和特异性,其作为快速、准确的检测手段,给临床提供诊断依据,控制结核感染蔓延新法。方法选择本院门诊及住院的结核病患者289例,分别进行T-SPOT.TB及结核抗体IgM检测。结果外周血检测T-SPOT.TB阳性263例,结核抗体IgM检测阳性243例,对照阳性率:T-SPOT.TB检测阳性率为91.0%(263/289),结核抗体IgM检测阳性为84.1%(243/289),经统计学处理P<0.05,T-SPOT.TB检测优于结核抗体IgM检测。T-SPOT.TB优于结核抗体IgM检测,T-SPOT.TB检测诊断,可在保持高灵敏性,高特异性的同时,进一步提高结核分枝杆菌检出率。结论对早期诊断可获得较好治疗效果的肺结核和肺外结核患者,该选择灵敏度高,特异性强,T-SPOT.TB快速检测有一定优势。
Objective To analyze the sensitivity and specificity of T-SPOT.TB in test of tuberculosis. As the fast and accurate examination means, it provided diagnostic criteria in clinic and new method to control the spread of tuberculosis. Methods Two hundred and eighty-nine cases of outpatient and inpatient TB patients, respectively, were detected in T-SPOT.TB test and tuberculosis antibody IgM. Results Peripheral blood T-SPOT.TB test was positive in 263 cases, tuberculosis antibody IgM were detected in 243 cases. The positive rate: T-SPOT.TB test was 91.0% (263/289), tuberculosis antibody IgM test was 84.1% (243/289). After statistics processing. P 〈 0.05. T-SPOT.TB test was better than tuberculosis antibody IgM test. T-SPOT.TB test was better than tuberculosis antibody IgM test, T-SPOT.TB test can not only maintain the high sensitivity and high specificity, but also further improve the positive rate of mycobacterium tuberculosis. Conclusion Pulmonary tuberculosis and extrapulmanory tuberculosis patients, who can obtain a good therapeutic effect if they are give early diagnosis, should select the method with high sensitivity and strong specificity. So T-SPOT.TB rapid test has certain advantage.
