摘要
目的了解一次性20ml注射器穿刺骨髓腔输液在实施心肺复苏急救的成人创伤中的应用效果。方法根据实验设定的胫骨骨髓腔输液的纳入标准,采用队列研究的方法,2010年1月~2011年12月实施心肺复苏急救的成人创伤患者62例,分为实验组和对照组。实验组共计27例,首选胫骨骨髓腔输液通;对照组35例首选外周静脉输液通道补液。比较两组通道建立的时间、首次给药时间、补液速度,采用成组t检验。结果两组通道建立的时间、首次给药时间比较有明显差异(P〈O.05)。两组补液速度比较:骨髓腔输液通道在未加压情况下,与外周静脉补液速度有差异(P〈0.05);在加压的情况下,与外周静脉输液通道补液速度差异不大(P〉0.05)。结论在成人创伤实施心肺复苏术的院外及院内急救中,若外周静脉通道难以建立,采用一次性20ml注射器穿刺胫骨骨髓腔输液可达到快速给药、加压快速输液的目的。
Objective To investigate the 20ml disposable syringe aspiration of bone marrow cavity infusion in the implementation of cardiopulmonary resuscitation in the adult trauma application. Method According to the experimental setting of the tibia[ bone marrow cavity infusion into the standard, using a cohort study method, to our department in 2010 January -2011 December implementation of cardiopulmonary resuscitation in the adult trauma patients with 62 cases, divided into experimental group and control group. The experimental group was 27 cases, the preferred tibia[ bone marrow cavity infusion through; the control group of 35 cases of peripheral intravenous infusion fluid channel preferred. Comparison of two groups of channel establishment time, the first delivery time, rehydration rate, with group t test.Result Two groups of channel establishment time, the first delivery time have obvious difference ( P〈0.05 ). Two groups of rehydration speed comparison: bone marrow cavity infusion channel in case of not pressurized, and peripheral vein infusion speed difference (P〈0.05); in the absence of pressurization, and peripheral vein infusion channel infusion speed difference (P〉0.05). Conclusion In the adult trauma cardiopulmonary resuscitation in prehospital and hospital emergency, if peripheral venous access is difficult to establish, using 20ml disposable syringe needle tibial bone marrow cavity infusion can achieve rapid delivery, pressurized rapid infusion of purpose.
