摘要
目的通过测定 HBeAg阳性慢性肝炎(CHB)血清 HBV- DNA水平,判断干扰素在病情治疗中的作用。方法 28例HBeAg阳性 CHB接受干扰素治疗。血清 HBV- DNA浓度测定应用 AC-9600 Amplisensor荧光 PCR定量系统,测定范围定为10300-950copies/mL。结果 28例 HBeAg阳性 CHB患者治疗结束时呈完全应答(CR)者 13例,无应答(NR)15例。对 13例 CR患者继续随访6个月,其中4例血清ALT再升高(复燃)。血清HBV-DNA水平在治疗前,CR不伴ALT复燃组、CR伴ALT复燃组和NR组之间无差异;治疗结束时,CR不伴 ALT复燃组 HBV- DNA为 10344 043copies/mL,明显低于 CR伴 ALT复燃组(10684 051copies/mL)和NR组(10718 066copies/mL)。结论在HBeAg阳性CHB病例,干扰素治疗结束时,检测HBV-DNA水平对于判断疗效有指导价值,HBV- DNA低于 104copies/mL往往能取得持久疗效。

(Molecular biology laboratory Luohu people's hospital , Shen zhen 518001) Abstracg:Aim To measure serum hepatitis B virus(HBV)-DNA levels in HBeAg positive patients, and estimate the efficacy of interferon in treating chronic hepatitis B. Methods Twenty eight HBeAg-positive CHB patients were treated with interferon. Quantitative analysis of HBV-DNA was carried out using AG-9600 PCR system. The range of the measurement was 10 3-9.5 copies of genome/mL in serum. Results Among patients heated with interferon 13 eases had complete response(CR), and 15 no response(NR). Among 13 CR, a serum ALT flare-up during 6 months follow-up was observed in 4 patients. Before treatment , serum HBV-DNA in CR without ALT flare-up, CR with ALT flare-up and NR were similar. At the end of treatment serum HBV-DNA in CR without ALT flare-up( copies/mL)was significantly lower than that in CR with ALT flare-up (copies/mL)and NR(copies/mL). Conclusions Measurement of serum HBV--DNA levels is helpful for differentiation of various HBV infectious conditions and for prediction of interferon efficacy in HBeAg-positive chronic CHB pa- tients. Serum HBV-DNA below 104 copies/mL is an important marker for successful heatment.
