摘要
目的分析阴道彩超在异位妊娠诊断中的图像特征,进一步提高诊断符合率。方法分析淮南朝阳医院经彩超诊断、手术或病理证实的36例异位妊娠患者的阴道彩超诊断结果和图像特征。结果 36例患者中,卵巢妊娠1例,表现为附件区不均质包块,与卵巢不可分;输卵管间质部妊娠1例,表现为孕囊自左宫角处向外膨出,其外侧缘无明显的肌壁包裹,输卵管壶腹部妊娠25例,表现为输卵管局部增粗,与子宫无直接相连;输卵管峡部妊娠4例,表现为孕囊紧邻宫角外侧,输卵管伞部妊娠5例,表现为包块距宫体较远,盆腔中至大量积液。结论各型异位妊娠在图像上具有不同的声像学表现,超声科医师应仔细分析经阴道超声的图像,结合临床资料提高诊断异位妊娠的准确性。
Objective To analyze the image features of vaginal color Doppler ultrasound in the diagnosis of ectopic pregnancy, and to further improve the rate of diagnosis. Methods Thirty - six cases of eetopic pregnancy were diagnosed by vaginal color Doppler and con- firmed by surgery or pathology. The results and image features were analyed. Results One case was ovarian pregnancy, whose attachment ar- ea was not homogeneous mass that could not be separated with ovarian; one case was interstitial tubal pregnancy, in which the gestational sac bulged outward from left and right of the uterine horn, whose outer edge was not wrapped by the muscle wall. There were 25 cases of tubal the ampulla pregnancy, whose pictures showed the partial thickening in tubal, which was not connected to the uterus directly ; four cases were fal- lopian tube isthmus pregnancy, which showed gestational sac close to the outside of the left and right uterine horn; there were five cases of pregnancy in tubal fimbria, whose figure showed the mass was far from uterus and a lot of fluid was found in pelvic. Different images had dif- ferent features which need careful diagnosis. Conclusion The uhrasonographic image performance of each type of ectopic pregnancy should be carefully analyzed by the ultrasonic doctors, and clinical history should be combined to further improve the accuracy of diagnosis of ectopic pregnancy
