防放射线手术操作房与传统铅衣、铅围脖和铅屏组合在防护效率与现场操作评价方面的对比研究 被引量:5

摘要 目的比较新型专利装置防放射线手术操作房与传统铅衣、铅围脖和铅屏组合在防护效果和现场操作评价方面的差别。方法在30台冠状动脉介入手术过程中同时使用防放射线手术操作房与传统的铅衣、铅围脖和铅屏组合防护系统,以盖革计数器测量放射当量以评价二者效率的优劣性。铅衣和防护房测量位置均设定为离地面130cm,相当于身高170cm术者的胸部位置,铅衣和防护房内外表面各设一测量点。屏蔽效率计算公式为:屏蔽效率=(外部X线当量-内部X线当量)/外部X线当量×100%。术者在使用过程中防护装置对手术的影响、球管调整时可操作性及舒适性进行评价打分。结果防放射线手术操作房在屏蔽效率(98.10%±0.80%比90.18%±1.70%,t=23.05,P<0.01)和舒适性(3.00±0比1.50±0.82,t=29.00,P<0.001)方面明显超过铅衣、铅围脖和铅屏组合,而在对手术的影响(0比0,P>0.05)、球管调整时的可操作性(2.93±0.25比3.00±0,P>0.05)方面两者无统计学意义。结论防放射线手术操作房对手术无不良影响,球管调整时操作灵活,其防护效果和舒适性明显优于目前的铅衣、铅围脖和铅屏组合。 Objective To compare the radiological protection effects and on-site operation evaluation between a new anti-radiation surgery operating room and combination of traditional lead rubber apron, collar and hanging screen. Methods The anti-radiation surgery operation room and the combination of lead rubber apron, collar and hanging screen were used together in 30 coronary interventional operations. Geiger counters were used to measure the radiological shielding effectiveness of the two radiological protection systems. For both systems, the geiger counters were set at 130-centimeter high points separately on the inner and outer surface of protective devices, which are equivalent to the level of a 170-centimeter tall doctor's chest. The shielding effectiveness is calculated as the following formula : shielding efficiency = ( outer X-ray dose - inner X-ray dose) / outer X-ray dose ~ 100%. And scores of the hampering to operation, flexibility in tube-directing and comfortability of the two systems were independently evaluated by the operators. Results Compared with the combination of rubber apron, collar and hanging screen, the anti-radiation interventional operation room is superior in shielding efficiency (98.10 ±0. 80% vs. 90. 18 ± 1.70% ,P 〈 0. 001) and comfortability(3.00 ±0 vs. 1.50 ±0. 82,P 〈0. 001), and equivalent in hampering to operation (Ovs. 0,P 〉 0. 05 ) and flexibility in tube-directing ( 2.93 ± 0. 25 vs. 3.00 ± 0, P 〉 0. 05. Conclusions Compared with the current combination of rubber apron, collar and hanging screen, the anti-radiation interventional operation room is superior in shielding efficiency and comfortability, and equivalent in hampering to operation and flexibility in tube-directing.
