摘要
目的 观察肥胖儿童早期血管内皮功能改变的特点。方法 收集 7~ 14岁单纯性肥胖儿童与正常对照儿童各 40例 ,分别测定其血浆内皮素 (ET)、6 酮前列环素 (6 K PGF1α)及血清一氧化氮 (NO)水平 ,并采用高分辨率多普勒超声测定肱动脉在静息、反应性充血和舌下含服硝酸甘油后血管舒张末期内径的变化。结果 肥胖儿童较正常儿童ET升高 ,6 K PGF1α和增加血流介导的血管舒张幅度 (FMD)均降低。FMD与NO和 6 K PGF1α呈正相关 ,而与ET呈负相关。结论 单纯性肥胖儿童血管内皮分泌功能紊乱 ,内皮依赖性舒张功能障碍 。
Objective To study the vascular endothelial dysfunction in early stage of obese children by detection of endothelium derived secretion levels and endothelium dependent relaxing response to flow mediated dilation (FMD). Methods The study group included 40 simple obese children aged from 7 to 14 years old. They were divided into two groups by obese index BMI, 25~30 BMI as group I, and BMI >30 as groupⅡ. Forty healthy children selected by paired investigation acted as a control group. Plasma ET and 6 k PGF 1α levels were assayed with RIA technique, the concentration of serum NO was determined with nitratase method. The arterial endothelium relaxing response was observed with Celermajer method by using high resolution Doppler ultrasound for measuring diameters at rest, after reactive hyperemia and after sublingual nitroglycerin (an endothelium independent dilator). Results The levels of NO and 6 k PGF1α were evidently decreased, while ET level was higher in study group than in control group. And the FMD was significantly weakened in study group. All the changes mentioned above were more remarkable in obese group II than in obese group I. The arterial dilation after sublingual nitroglycerin was similar between obese and healthy children. FMD was negatively correlated with BMI, SBP, DBP and ET, while positively correlated with NO and 6 k PGF1α. Conclusion In simple obese children there were dysfunctions of endothelial secretion and endothelium dependent relaxation. The later was correlated with the decrease of endothelium derived relaxing factors and the increase of contracting factors.
