摘要
目的在人工耳蜗植入者中验证开放式言语评估系统短句材料的表间等价性并加以改进。方法入选30个人工耳蜗植入者(年龄26.53±10.42岁,人工耳蜗使用时间22.81±21.73月),在声场中测试其在安静环境下开放式言语评估系统中10张音位平衡句表的言语识别率,并将原10个句表按所对应的言语识别率的均值大小合成5张新句表计算其言语识别率,采用RM-ANOVA和Tukey测试分析其表间等价性。结果 10个句表测得的言语识别率依次为59.43%±29.69%、55.15%±29.11%、56.52%±31.64%、50.14%±28.58%、52.57%±30.05%、52.43%±29.63%、50.62%±31.31%、49.57%±29.23%、54.47%±30.37%、49.91%±30.86%,10个句表之间的平均言语识别率差异有统计学意义[F(9,261)=7.443,P<0.001],将10个句表组合成5个新句表后,言语识别率依次为54.50%±29.10%、53.22%±31.03%、52.65%±28.55%、52.54%±30.48%、52.50%±29.36%,5个新句表之间等价性好[F(4,116)=0.941,P=0.443]。结论言语测听材料在正常听力人群中经过等价性验证后,仍然有必要在目标人群中进一步验证;开放式言语评估系统中的短句材料经过整合改进后,同时满足了言语评估所必需的音位平衡和表间等价性;相较于原句表,改进后的言语评估材料能为人工耳蜗植入者提供更准确的听觉言语评估。
Objective To evaluate the inter-list equivalency of the sentence materials for Mandarin Speech Perception test (MSP) and modify it by measuring the speech recognition score o{ cochlear implant (CI) users. Meth- ods According to Latin-Square design,sentence recognition scores(quiet background) were measured for each of the 10 phonemically balanced lists in 30 Mandarin speaking CI users(aged 26. 53 ± 10.42 years, 22.81± 21. 73 months for CI use time) in sound field. RM-ANOVA was administered to confirm the list equivalency. Results The speech recognition scores were 59.43%0±29.69%,55.15%0±29.11%,56.52%±31.64%,50.14%±28.58%, 52.57%±30.05%,52. 43% ± 29. 63%, 50. 62%±31. 31%,49. 57%± 29. 23%, 54. 47%± 30. 37%,49. 91%± 30.86%, respectively for the 10 lists. There were significance differences in sentence recognition scores across the 10 sentence lists [F(9,261)=7. 443, P〈0. 001]. While the speech recognition scores were 54.50%±29.10%,53. 22%±31.03% ,52.65%±28.55% ,52.54%±30.48% ,52.50%±29.36%, respectively after the lists being as- sembled into 5 new lists with 20 sentences in each list, and they were all equivalent[F(4,116)=0. 941, P= 0. 443]. Conclusion It is necessary to validate the list equivalency of testing materials for speech audiometry in cochlear im- plant users, even after the validation in normal hearing listeners. Compared with the original version, the modified sentence materials of mandarin speech perception test may be useful and more accurate for assessing speech recogni- tion performance of Mandarin-speaking CI users.
