摘要
目的评价18F-脱氧葡萄糖(18F-FDG)PET/CT(正电子发射计算机断层扫描)诊断肝内胆管细胞癌的价值.方法分析肝内胆管细胞癌行PET/CT检查病例16例,其中病理诊断组6例,临床诊断组10例.根据18F-FDG摄取程度将病灶分为3种类型.Ⅰ级分化程度高,低度恶性或典型类癌;Ⅱ级分化程度一般,中度恶性非典型类癌;Ⅲ级分化程度低,高度恶性小细胞癌.结果临床诊断中,2例为Ⅰ级低度恶性或典型类癌,2例为Ⅱ级中度恶性非典型类癌,2例为Ⅲ级高度恶性小细胞癌;病理诊断中,低度恶性或典型类癌5例,中度恶性非典型类癌3例,高度恶性小细胞癌2例.结论 18F-FDG PET/CT是诊断发病率较低的肝内胆管细胞癌的一种有效方法,能评价肝内胆管细胞癌的分化程度.
Objective To evaluate the value of18F-2-fluoro-2-deoxy-D-glucose(18F-FDG) PET/CT in diagnosis for patients with intrahepatic cholangiocarcinoma(ICC).Methods The PET/CT imaging of 16 patients who were diagnosed as ICC by 18 F-FDG PET/CT,including 6 cases got from clinical diagnosis and 10 cases from pathologic diagnosis.The tumors were divided into the following three types according to situation of18F-FDG uptake.Type Ⅰ was well-differentiated,low-malignant or typical carcinoid;type Ⅱ was normal-differentiated and mid-malignant atypical carcinoid;type Ⅲ was low-differentiated and well-malignant small cell carcinoma.Results During clinical diagnosis cases,2 cases were typeⅠ,1 case was typeⅡ,and 1 case was type Ⅲ.During pathologic diagnosis cases,5 cases were low-malignant or typical carcinoid,3 cases were mid-malignant atypical carcinoid,and 2 cases were well-malignant small cell carcinoma.Conclusions 18F-FDG PET/CT appeared to be a valuable method for the diagnosis and discrimination of ICC from benign focal lesion.
