微创钻孔引流治疗高血压脑出血临床体会 被引量:7

摘要 目的探究微创钻孔引流术在高血压脑出血患者治疗中的安全性、可行性及有效性。方法选取我院神经外科2010~2011年间采用微创钻孔引流治疗高血压脑出血的88例患者为研究对象,在CT下定位脑出血位置,尽量选择无血管、无功能区钻孔引流,同时使用尿激酶灌注血肿部位辅助血肿的溶解。观察患者治疗期间的病死率、住院时间及治疗后日常生活能力(ADL)等,并随访0.5~1年,了解患者院外情况。结果本组88例参与治疗的患者,总死亡率12.5%(11/88);平均意识恢复时间(11.2±3.8)h;术后3月ADL评分1级患者9例,2级患者48例,3级患者18例,4级患者3例,5级患者1例,恢复期死亡3例。结论对于高血压脑出血的患者早期使用CT定位下的微创钻孔引流术安全可靠,可以显著提高患者的生存率,降低致残率,有效改善患者术后生活质量,远期效果也良好,应积极推广临床使用。 Objective Explore the safety, feasibility and effectiveness minimally invasive drilling drainage treatment of hypertensive intracerebral hemorrhage. Methods Select hospital 2010N2011 minimally invasive drilling drainage in the treatment of 88 patients with hypertensive cerebral hemorrhage as the research objective,Observed in patients mortality, hospitalization time, and after treatment activities of daily living (ADL), and follow-up of 0.5~1 years, to understand the patient outside the hospital. Results 88 cases in this group of patients involved in the treatment, the total mortality rate of 12.5%(11/88); average awareness restore time (11.2 _+ 3.8) hours; ADL score after March 1 of 9 patients, 2 of 48 patients the class 18 patients, three cases of four patients, five patients one cases convalescence and 3 died. Conclusion Early use of CT positioning minimally invasive drilling drainage is safe and reliable, can significantly improve survival, reduce morbidity, and effectively improve the quality of life of patients, the long-term effect is also good.
