摘要
目的比较5种估算肾小球滤过率(eGFR)公式在评价老年住院患者肾功能情况的应用价值。方法分析年龄≥65岁577例的住院患者资料,以慢性肾病流行病学合作研究组(CKD-EPI)公式、肾病饮食调节(MDRD)公式、简化MDRD公式、中国改良MDRD公式和瑞金公式eGFR,比较各种公式在评估老年患者肾功能的差异。结果共收集577例患者资料,年龄65-97岁,平均年龄(78.8±5.7)岁。上述公式计算的eGFR值分别为(64.7±19.5)、(68.6±22.9)、(71.6±24.2)、(88.3±29.8)和(65.9±18.6)ml/(min.1.73m2)(F=97.468,P=0.000)。而各公式计算的肾功能下降发生率分别为38.0%、33.3%、29.6%、14.7%和35.4%(x2=91.925,P=0.000)。相关分析显示,上述公式的eGFR值与年龄的相关系数(r)分别为-0.320、-0.253、-0.228、-0.228和-0.252(P=0.000)。结论老年住院患者中,eGFR值均随增龄呈下降趋势,而肾功能下降发生率随增龄而增高。与其他公式比较,CKD-EPI公式低估了eGFR,而高估了肾功能下降发生率,其估算的eGFR值与年龄相关性最高。
Objective To compare the value of five equations for estimated glomerular filtration rate(eGFR) applied in aged inpatients.Methods The clinical data of inpatients aged 65 years or older were collected.The eGFR values to evaluate renal func tion were respectively calculated by Chronic Kidney Disease Epidemiology Collaboration(CKD-EPI) equation,Modification of Diet in Renal Disease(MDRD) equation,abbreviated MDRD equation,modified MDRD equations for Chinese and Ruijin equation for Chinese.Results In all 577 subjects aged(78.8±5.7) years old,the mean eGFR values were(64.7±19.5)、(68.6±22.9)、(71.6 ±24.2)、(88.3±29.8) and(65.9±18.6) ml/(min.1.73m2)(F=97.468,P=0.000) with above-mentioned equations,while the incidence rates of impaired renal function were separately 38.0% 、 33.3% 、 29.6% 、14.7% and 35.4%(x2=91.925,P=0.000).Meanwhile,the eGFR values with previous equations were equally negative correlated with age,while the correlative coefficients were-0.320、-0.253、-0.228、0.228 and-0.252(P=0.000) respectively.Conclusions Regardless of calculated by different equations,the eGFR values decreased in elderly patients with aging.Compared with other equations,the CKD-EPI equation underestimated eGFR and overestimated the in cidence rates of impaired renal function in aged patients,while the eGFR value with CKD-EPI equation was better correlated with age.
