摘要
目的探讨三维彩色多普勒超声在慢性肾脏病的诊断中的价值。方法选择我院自2008年12月—2011年12月临床诊断的慢性肾脏病患者90例,其中1期22例,2期18例,3期20例,4期14例,5期16例。另选取同期体检正常的健康者30例为对照组,采用型号为GE Voluson730彩色多普勒超声诊断仪,检测和计算受检者叶间动脉阻力指数(RI)、血管指数(VI)、三维肾脏体积、二维肾脏体积、皮质厚度。结果 1期组较对照组三维肾脏体积、二维肾脏体积升高,差异均有统计学意义(P<0.05);2期组较对照组VI、三维肾脏体积降低,较1期组VI、三维肾脏体积、二维肾脏体积降低,差异均有统计学意义(P<0.05);3期组较对照组、1期组、2期组RI升高,VI降低,差异均有统计学意义(P<0.05);3期组较对照组三维肾脏体积降低、二维肾脏体积无明显变化;4期组较对照组、1期组、2期组RI升高,VI、二维肾脏体积、皮质厚度降低,差异均有统计学意义(P<0.05);5期组较对照组、1期组、2期组、3期组RI升高,VI、三维肾脏体积、二维肾脏体积、皮质厚度降低,差异均有统计学意义(P<0.05)。结论 VI能敏感体现早期慢性肾脏病肾功能异常,三维彩色多普勒超声可作为诊断慢性肾脏病直观、简便的一种检查方法。
Objective To study the value of three - dimensional ultrasound in chronic kidney disease (CKD). Meth- ods 90 CKD patients admitted to our hospital from December 2008 to December 2011 were selected. Among the patients, 22 ca- ses were in stage one, 18 in stage two, 20 in stage three, 14 in stage four and 16 in stage five. Meanwhile, 30 healthy people were involved into the study as control group. The renal volume and VI were calculated by the application of three - dimensional ultrasound, cortical thickness (CT), resistance index (RI) and the volume of kidney were measured with two -dimensional ul- trasound. Results The three and two dimensional renal volumes were significantly increased in stage - one group compared with control group ( P 〈 0.05 ) . The YI and three - dimensional renal volume were significantly decreased in stage - two group com- pared with control group, while the VI, three and two dimensional renal volume in stage - two group were significantly decreased compared with stage - one group ( P 〈 0. 05 ) . Compared with control group, stage - one group and stage - two group, the RI and VI in stage - three group were significantly increased and decreased respectively (P 〈 0. 05) . Compared with control group, the three and two dimensional renal volumes were decreased or without changing respectively in stage - three group. Compared with control group, stage - one group and stage - two group, the RI was significantly increased, the VI, two - dimensional renal vol- ume and cortical thickness were significantly decreased in stage - four group ( P 〈 0.05 ) . Compared with control group, stage - one group, stage - two group and stage - three group, the RI was significantly increased, the VI, three and two dimensional re- nal volume and cortical thickness were significantly decreased in stage - five group (P 〈 0. 05 ) . Conclusion Three and two di- mension ultrasound of renal volume and VI values can sensitively found the abnormalities in chronic kidney disease, therefore, three - dimensional power ultrasound can be used as an intuitive and easy screening method for the diagnosis of chronic kidney dis- ease.
