脑钠肽对慢性心力衰竭患者预后评估的系统评价 被引量:1

Systematic review of brain natriuretic peptide for prognosis in patients with chronic heart failure

摘要 目的系统评价脑钠肽对慢性心力衰竭(Chronic heart failure,CHF)患者预后的评估。方法计算机检索Web of Science、OVID、PubMed、EMbase和Cochrane图书馆等电子数据库,查找脑钠肽对CHF预后评估的队列研究和病例对照研究,检索时限从建库至2012年10月,并手工检索相关会议文献及未公开发表的相关文章。对符合条件的RCT,由3位评价员独立进行资料收集、质量评估以及相互核对后,采用RevMan 5.1软件进行Meta分析。结果共纳入5个队列研究,共484例患者。Meta分析结果显示,CHF患者心血管事件组(死亡或因CHF再住院)的BNP较非事件组高,差异有统计学意义[WMD=411.25,95%CI(210.93-611.57),P<0.01]。CHF患者死亡组的BNP较存活组高,差异有统计学意义[WMD=342.65,95%CI(310.89-374.42),P<0.01]。观察经治疗4个月、6个月的CHF患者心血管事件组的BNP仍较非事件组高,差异有统计学意义[WMD=344.68,95%CI(312.74-376.62),P<0.01]。说明CHF患者BNP增高者更易发生心血管事件。结论现有证据表明,BNP可作为CHF患者发生心血管事件的预测指标。 Objective To evaluate the brain natriuretic peptide in the assessment of the prognosis of patients with chronic heart failure (chronic heart failure, CHF). Methods By computer retrieve Web of of Science, OVID, PubMed, EMbase and Cochrane Library electronic database, look for brain natriuretic peptide CHF prognostic assessment of cohort and case-control study, the retrieval time limit from building a database to October 2012, and manual searches related conference literature and unpublished articles. Qualifying RCT data extracted independently by three reviewers, quality assessment and cross-checking, Meta-analysis using RevMan 5.1 software. Results A total of five cohort study of 484 patients. Meta-analysis showed that the group of CHF patients with cardiovascular events (death or rehospitalization for CHF) BNP than non-high event group, the difference was statistically significant WMD = 411.25, 95%CI (210.93-611.57), P 〈 0.01 . Group of death BNP in CHF patients than survival group,the difference was statistically significant WMD = 342.65,95% CI (310.89-374.42),P 〈 0.01. Observed after treatment in April, June, CHF patients with cardiovascular events group BNP still non-event group, the difference was statistically significant WMD = 344.68, 95%CI (312.74-376.62), P 〈 0.01. CHF patients with BNP increased more susceptible to cardiovascular events. Conclusion The available evidence suggests, BNP can be used as a predictor of cardiovascular events in patients with CHF.
