乙酰半胱氨酸泡腾片预防非小细胞肺癌放射性肺炎的疗效观察 被引量:6

Effect of acetylcysteine effervescent tablets on the prevention of radiation pneumonitis in nonsmall-cell lung carcinoma patients

摘要 目的 观察乙酰半胱氨酸泡腾片对局部晚期非小细胞肺癌(NSCLC)患者放疗后放射性肺炎发生的干预作用.方法将64例局部NSCLC患者随机分为对照组和治疗组,对照组32例单纯行局部三维适形放疗(3DCRT),治疗组32例从3DCRT第1天开始口服乙酰半胱氨酸泡腾片,放疗结束后继续口服3个月.观察放疗前、放疗后3个月两组患者临床症状、胸部CT、KPS评分及血浆谷胱甘肽(GSH)水平.结果 治疗组放射性肺炎发生率为21.8%,低于对照组(37.5%),治疗组放疗后血浆GSH含量为(10.15±0.67) μmol/L,高于对照组[(4.97±0.36)μmol/L],且治疗组的生存质量优于对照组,两组比较差异均具有统计学意义(P<0.05).结论 乙酰半胱氨酸泡腾片能提高血浆中GSH含量,降低放射性肺炎发生率,改善患者放疗后的生活质量. Objective To investigate the effect of acetylcysteine effervescent tablets on the prevention of radiation pneumonitis in advanced non small cell lung carcinoma(NSCLC) patients treated with radiotherapy. Methods Sixty-four patients with advanced NSCLC were randomly assigned to control group (received only 3 DCRT, n = 32 ) or therapeutic group (3DCRT plus acetylcysteine, n = 32 ). The patients in therapeutic group were treated with acetylcysteine effervescent tablets in the beginning of 3DCRT,and continued another 3 months after 3DCRT ended. The clinical symptoms, CT of thorax, KPS scores and the plasma GSH level were evaluated. Results In radiation plus acetylcysteine group and in control group, the incidence of radiation pneumonitis was 21.8 % vs. 37.5 % ;and the plasma concentration of GSH was ( 10.15 -+0.67) μmol/L vs. 4.97 ±0. 361zmol/L. The quality of life and KPS scores were im- proved in radiation plus acetylcysteine group. Conclusion Acetylcysteine effervescent tablets can raise plasma concentration of GSH and reduce the incidence of radiation pneumonitis, thus can improve the quality of life after radiotherapy in advanced NSCLC patients.
