摘要
目的评估射频消融序贯^(125)I粒子植入方法治疗恶性实体肿瘤的近期疗效和安全性。方法回顾性分析2009年8月—2012年12月广东省中医院肿瘤科经皮射频消融序贯^(125)I粒子植入治疗14例恶性肿瘤患者的临床资料,并分析短期疗效(目标肿瘤控制率)及安全性。结果经皮射频消融治疗术后复查手术病灶影像,14例患者共21处病灶肿瘤仍有活性,且患者可耐受进一步局部治疗,遂行^(125)I粒子植入治疗。14例患者肿瘤中位直径5.8(2.0～7.8)cm,治疗均获成功,无治疗相关死亡,主要并发症为胆心反射2例,发热9例,所有患者射频消融时均有治疗相关疼痛,轻度疼痛10例(71.4%),中重度疼痛4例(28.6%)。目标肿瘤完全缓解(CR)率为35.7%(5/14),部分缓解(PR)率50.0%(7/14),中位无疾病进展时间(PFS)为12个月,中位生存时间(OS)为30个月。1年目标肿瘤控制率(CR+PR)为60.0%,1年生存率为70.0%。结论对直径较大(>5 cm)或特殊部位的肿瘤,经皮射频消融序贯^(125)I粒子植入是安全、有效的,值得进一步研究。
Objective To assessment the efficacy and safety o（percutaneous radiofrequency ablation（RFA） followed by 125^I seed implantation for advanced cancer.Methods Retrospectively studied 14 consecutive patients treated with RFA followed by 125^Iseed implantation who were admitted in Oncology Department of Guangdong Provincial Hospital of TCM between August 2009 and December 2012.Treatment related complications,response of tumors,progression free survival of treated tumor（PFS） and overall survival were assessed.Results A total of 89 patients with accepted RFA for treatment.After the assessment ol image study,14 patients still have tumors with alive and then accepted125^Iseed implantation.Correct place-meat of the ablation device and 125^I seed into the target tumor with completion of the planned treatment protocol was feasible in all of the 14 patients.No procedure related deaths occurred.The diameter of treated tumors various from 2.0 to 7.8 cm（median 5.8cm）.Major complications were gallbladder cardiac reflex in 2 patients and fever in 9 patients.All of the patients suffered from treatment associated pain,10（71.4%） patients with mild pain and only 4（28.6%） patients with moderate to serious pain.Complete response of target tumors was seen in 5 patients（35.7%）,and partial response was seen in 7 patients （50.0%）.Median overall survival（OS） was 30 months,and median progression free survival（PFS） was 12 months.Con firmed response rate of the treated tumor lasting at least 1 year was 60.0%.Overall survival rate of 1 year was 70.0%.2 patients were cured.Conclusion For large（diameter more than 5 cm） or special located tumor,RFA followed by 125^Iseed implantation for advanced cancer is effect and safe with minimal damage and few complications.
