148例儿童肠球菌性尿路感染临床特点与耐药性分析 被引量:11

摘要 目的分析儿童肠球菌性尿路感染的临床特点,了解其病原菌的分布及耐药性,为临床预防与治疗儿童肠球菌属感染提供依据。方法对住院的肠球菌性尿路感染148例患儿临床资料进行回顾性分析。结果病原菌以屎肠球菌和粪肠球菌为主,两者对抗菌药物均有比较高的耐药性,屎肠球菌对各种抗菌药物的耐药率明显高于粪肠球菌;屎肠球菌检出耐青霉素菌株68株,对青霉素耐药率为79.1%;检出对呋喃妥因、万古霉素、利奈唑胺耐药菌株分别为4、4、2株,耐药率分别为4.7%、4.7%、2.3%;多药耐药菌株检出率85.7%;粪肠球菌检出耐青霉素菌株10株,耐药率为16.7%;检出对呋喃妥因、利奈唑胺耐药菌各1株,耐药率为1.7%;未检出耐万古霉素粪肠球菌;粪肠球菌多药耐药菌株检出率67.2%;婴幼儿、肾脏疾病以及过度使用抗菌药物及免疫抑制剂是医院感染、主要易患因素。结论肠球菌属是导致尿路感染的较常见致病菌,临床表现无明显特异性,耐万古霉素及耐利奈唑胺肠球菌属有增加趋势,多药耐药现象严重。 OBJECTIVE To analyze the clinical features of Enterococcus urinary tract infections in children and to explore the distribution and drug resistance of pathogens so as to guide the clinical medication. METHODS The clinical data of 148 hospitalized patients with Enterococcus urinary tract infections in the hospital was analyzed retrospectively. RESULTS The main species of Enterococcus were E. faecium and E. faecalis, both of which had relatively high resistance to antimicrobial drugs. E. faecium had higher resistance rates than E. faecalis. There were 68 (79.1%) strains of penicillin-resistant E. feces isolated; there were 4 (4.7 %) strains of nitrofurantoin-resistant isolates, 4 (4.7 %) strains of vancomycin-resistant isolates, and 2 (2.23 % ) strains of linezolid-resistant isolates; the detection rate of the muhidrug-resistant strains was 85.7 % ; there were 10 (16.7^) strains of peni-cillin-resistant E. faecalis isolated; there were 1 (1.7%) strain of nitrofurantoin-resistant isolate and 1 (1.7%) strain of linezolid-resistant isolate; there were no strains of vancomycin-resistant E. faecalis isolated; the detec-tion rate of the multidrug-resistant E. faecalis was 67.2%. The main predisposing factors included the nosocomi-al infections, young children, kidney disease and excessive use of antibiotics and immunodepressant. CONCLUSION Enterococci are the common clinical isolates of pathogens causing urinary tract infections, there are no typical clinical manifestations. The vancomycin-resistant Enterococci and linezolid-resistant Enterococci are increased, and the strains are highly multidrug-resistant.
