摘要
目的探讨随机尿、晨尿视黄醇结合蛋白(RBP)/肌酐(Cr)比值与24 h尿RBP、微量白蛋白(mAlb)的相关性及可替代性。方法选择2012年1~12月50例广西壮族自治区人民医院泌尿内科住院的早期肾功能损伤患者为受试组,正常体检者50例作为对照组,收集两组随机尿、晨尿及24 h尿液标本,进行其RBP、mAlb及尿Cr检测。结果与对照组比较,受试组随机尿、晨尿、24 h尿中mAlb[(283.93±10.29)、(279.31±20.39)、(2310.24±293.02)mg/L]、RBP[(3.91±1.02)、(3.74±1.00)、(26.86±3.40)mg/L]及RBP/Cr[(6.02±1.82)、(5.61±1.23)、(44.35±7.29)mg/g]均升高(P<0.01)。肾损伤患者随机尿及晨尿RBP/Cr与24 h mAlb、RBP呈一定的正相关,相关系数分别为:随机尿:r=0.664,r=0.813;晨尿:r=0.732,r=0.830。随机尿、晨尿液RBP/Cr对肾损伤判断的敏感度(81.2%、95.1%)、特异度(85.3%、90.8%)、阴、阳性预期值(88.1%、91.2%与80.2%、98.3%)均高于24 h尿检测值(78.3%、84.2%、78.0%、80.3%),三者比较差异有计学意义(P<0.05)。结论检测尿液RBP、mAlb能早期监测肾小球和肾小管的病变,可作为检测早期肾损伤的敏感指标;随机尿、晨尿RBP/Cr代替24 h尿检测更为方便,且灵敏度和特异性高于24 h尿检测结果。
Objective To study the correlation and the alternative of RBP/Cr ratio in random urine,morning urine and RBP and mAlb in 24 h urine.Methods From January to December 2012,50 cases with early renal damage in Uropoiesis Medical Department of People’s Hospital of Guangxi Zhuang Autonomous Region were chosen as test group,and 50 cases with normal physical examination were chosen as control group,two groups of random urine collection,morning urine and the 24 h urine specimens were collected,the RBP,trace albumin(mAlb) were detected.Results Compared with control group,test group of random urine,morning urine,24 h urinary mAlb [(283.93 ±10.29),(279.31± 20.39),(2310.24±293.02) mg/L],RBP [(3.91±1.02),(3.74±1.00),(26.86±3.40) mg/L] and higher RBP/Cr [(6.02 ±1.82),(5.61±1.23),(44.35±7.29) mg/g] were higher(P &lt; 0.01).Renal damage in patients with random urine and morning urine RBP/Cr and 24 mAlb,RBP had certain positive correlation,the correlation coefficient of random urine were r = 0.664,r = 0.813,the morning urine were r = 0.732,r = 0.830.Random urine,morning urine RBP/Cr on renal damage degree of judgment(81.2%,95.1%),sensitivity(85.3%,90.8%),specific,masculine and expected values(88.1%,91.2%,80.2%,98.3%) were more than 24 h urine detection value(78.3%,84.2%,78.0%,80.3%),the three compari son was statistically significant different(P &lt; 0.05).Conclusion Detection of urine RBP,mAlb monitoring early glomerular and renal tubular lesion,can be used for sensitive detection of early renal damage indicator;random urine,morning urine RBP/Cr can be instead of the 24 h urine testing,which is more convenient,and the sensitivity and specificity is better than the 24 h urine test.
