摘要
目的评价醋酸联合碘染色肉眼观察(VIA-VILI)在宫颈癌筛查中的应用价值。方法 2 836例妇科病患者,同时进行VIA、VIA-VILI、宫颈新柏氏薄层液基细胞学(TCT)检查,阳性病例及有高危因素、临床可疑病例进行阴道镜下多点活检病理组织学检查。结果 TCT异常检出率为19.76%(549/2 779),其中高度鳞状上皮内病变(HSIL)与低度鳞状上皮内病变(LSIL)分别为51例(1.83%)、114例(4.10%)。VIA、VIA-VILI阳性检出率分别为8.22%(233/2 836)、10.33%(293/2 836)。549例患者进行了阴道镜下多点活检病理检查,正常310例,LSIL/CINⅠ203例,HSIL/CINⅡ~Ⅲ34例,癌2例。细胞学检查的LSIL、HSIL及VIA、VIA-VILI敏感度分别为12.67%、8.53%、55.34%、70.05%;特异度分别为96.57%、98.66%、96.40%、94.13%;阳性预测值分别为24.56%、35.29%、60.09%、47.10%。结论 VIA-VILI能早期发现宫颈癌及癌前病变,有较高敏感度,并且该方法具有简单、经济、对设备要求很低等优点,对资源有限、经济不发达的农村地区具有较高的应用价值。
ObjeOive To assess the application of naked-eye visual inspection with acetic acid(VIA) and visual inspection with Lugol's iodine (V-ILl) to cervical cancer screening. Methods 2 836 patients with gynecological diseases underwent thinprep cytology test(TCT),VIA,and a combination of VIA and VILI. The positive cases,the suspected cases, and the cases of high-risk factors underwent colposcopic biopsy. Results The abnormal detection rate of TCT was 19.76% (549/2 779),and the eases of high-grade squar^ns intraepithelial lesion(HSIL),low-grade squamous intmepithelial lesion (LSIL) accounted for 51 ( 1.83% ), 114 (4.10%), respectively. The positive rates of VIA, VIA-VILI were 8.22% (233/2 836), 10.33% (293/2 836),respectively. Of 549 women underwent colposcopic biopsy,310 normal cases, 203 eases of LSIL/CIN I ,34 cases of HSII_,/CIN 11 - m and 2 cancer cases. Sensitivity of cytology was 12.67% for LSIL,8.53% for HSIL,55.34% for VIA and 70.05% for VIA-VILI. The corresponding specificities were 96.57%, 98.66% ,96.40% and 94.13%, respectively. The positive predictive values were 24.56%, 35.29%, 60.09% and 47.10% , respectively. Condtltsion VIA-VILl has a higher sensitivity for early diagnosis of cervical cancer and precancerous lesions,which is cheap,easy to use,and of lower equipment demands. It has a greater application value for the resoume-limited and undeveloped rural regions.
