摘要
目的:探讨痔病症状发作的诱发因素及其与中医证型的相关性。方法:应用病例对照研究方法,对符合要求的研究对象进行问卷调查,对临床资料进行整理并统计分析。结果:饮酒、辛辣饮食3～5次/周、排便频率≥14次/周、大便性状质硬、大便性状质稀、疲劳及水果蔬菜摄入量>500g/d与痔病症状发作有统计学关联且与中医证型构成比有差异。结论:饮酒、辛辣饮食、排便频率、大便性状、疲劳是痔病症状发作的诱发因素,水果蔬菜摄入量是保护因素。疲劳是痔脾虚气陷证型的诱发因素,辛辣饮食是湿热下注证型和气滞血瘀证型的诱发因素。
Objective To evaluate the causative factors for onset of hemorrhoidal symptoms, providing scien- tific evidences for prevention from hemorrhoids. Methods A hospital based case-control study was carried out. The self-designed questionnaire was used to collect the information on geographic characteristics, occupa- tion, life style, eating habit, bowel evacuation habit, prolonged sitting and fatigue. Pair Chi-square test test, Paired t-test and non-conditional logistic regression model were used, with univariate and multivariate to esti- mate odds rations （OR） and the confidence interval （95% CI）. Results The results of multivariate analysis showed that drinking, favouring spicy food, fruit and vegetable, frequency of bowel evacuation more than fourteen per week, softening of the defecate, hardening of the defecate, and fatigue were associated with onset of hemor- rhoid symptoms. The results of multivariate analysis showed that fatigue was associated with syndrome of Sinking Spleen Qi, favouring spicy food was associated with syndrome of Damp-heat Flowing type, favouring spicy food was assoeiated with syndrome of Qi Stagnation and Blood Stasis type, the quality of feces was associated with syndrome of Wind-heat Invading Intestine. Conclusion The results of univariate and multivariate analysis showed that drinking, favouring spicy food, fruit and vegetable, frequency of bowel evacuation, fatigue, softening of the defecate and hardening of the defecate may be the causative factors for onset of hemorrhoidal symptoms. The results of uuivariate analysis showed that favouring spicy food, frequency of bowel evacuation, the quality of feces, and fatigue were associated with the TCM syndromes.
