摘要
目的 探讨二磷酸盐对人工关节松动的影响。方法 随机选择准备行人工髋关节置换术患者 17例。采集外周血 ,分离单核细胞 ,每例标本分成 5份。A组 :仅单核细胞 ,为对照组 ;B组 :单核细胞 +骨水泥微粒 ;C组 :单核细胞 +骨水泥微粒 +阿可达 (5 μg/ml) ;D组 :单核细胞 +骨水泥微粒 +阿可达 (10 μg/ml) ;E组 :单核细胞 +骨水泥微粒 +阿可达 (2 0 μg/ml)。培养 48小时后 ,放免法测定细胞上清液中肿瘤坏死因子 (TNF)含量。结果 单核细胞 +骨水泥微粒组的TNF含量均明显高于对照组 (P <0 0 1) ;单核细胞 +骨水泥微粒 +阿可达组的TNF含量明显低于单核细胞 +骨水泥微粒组 (P <0 0 1)。说明骨水泥微粒可刺激单核细胞分泌TNF ,而二磷酸盐能有效地抑制单核细胞分泌TNF。结论 二磷酸盐能防治人工关节置换术后由TNF等溶骨性因子引起的假体周围的骨溶解 ,因此二磷酸盐有望成为防治人工关节松动的有效药物。
Objective To explore the effects of bisphosphonate on aseptic loosening of prothesis.Methods From 17 patients,blood samples were taken,whose mononuclear cells were isolated and then divided into five groups.Group A:only mononuclear cells,which was control group;group B:mononuclear cells+PMMA particles;group C:mononuclear cells+PMMA particles+Pamidronate(5μg/ml);group D:mononuclear cells+PMMA particles(10μg/ml);group E:mononuclear cells+PMMA particles(20μg/ml).They were co-cultured for 48 hours,and then the TNF was determined.Results The levels of TNF were much higher in group B than in group A,C,D and E( P <0 01).But there were no significant difference in group C,D and E( P >0 05).It shows the PMMA particles could stimulate mononuclear cells to excrete TNF,which can be inhibited by bisphosphonates.Conclusion By inhibiting bone resorption associated with the particles and TNF,Pamidronte and other bisphosphonates may have clinical utility in the prevention and treatment of aseptic loosening of prothesis.
