自体血浆分离联合术中自体血回收技术在骨科手术中对凝血功能的影响 被引量:4

Influence of autologous blood plasma separation combined with intraoperative autologous blood recovery technology's on blood coagulation function in orthopaedic surgery

摘要 目的比较自体血浆分离联合术中自体血回收技术在骨科手术中对凝血功能的影响。方法收集行骨科手术患者60例,分为自体血浆分离联合术中自体血回收组(A组)和单纯自体血回收组(B组),各30例。A组采用CellSaver5+型血液回收机回收术野出血,并经白细胞滤器回输,同时输入术前分离保存的血浆,B组采用Cell Saver5+型血液回收机回收术野出血,并经白细胞滤器回输。结果两组间在T0、T1、T2、T3、T4及T5时点的Hb、Hct及Plt差异均无统计学意义(均>0.05),A组自体血回输的同时输入术前保存的自体血浆后,PT和APTT均有所降低,在围手术期A组T3、T4、T5与B组差异均有统计学意义(均<0.05)。两组术中失血量和术中回输自体血量差异均无统计学意义(均>0.05);A组术后2例患者输注异体血,B组术后7例输注异体红细胞,差异有统计学意义(<0.05);A组术后24 h引流量及评价输血量与B组差异有统计学意义(均<0.05)。结论自体血浆分离联合术中自体血回收技术不增加失血量,不影响血小板功能,能很好的补充丢失的凝血因子,是一项值得推广的新技术。 Objective To observe the influence of autologous plasma separation combined with intraoperative autologous blood recovery technology in orthopedic surgery on blood coagulation. Methods Sixty eases underwent orthopedic surgery were randomly divided into two groups, 30 cases in each group. Group A were given with preoperative autologous plasma separation combined with intrmperafive autologous blood recovery, with Cell Saver type 5 + blood recycling machine used to recycle blood in the operation field, and the leukocyte filter back to Iransfuse, at the same time preoperative separation plasma was input. Group B were given with pure intraoperafive autologous blood which recycled by Cell Saver5 + blood recycling ma- chine and transfused by leucocyte filter. Results Between the two groups at TO, T1, T2, T3, T4 and T5 time point, Hb, Hct and Pit differences were not statistically significant (P 〉 0.05). PT and APTT decreased after transfusing blood in group A, and during the periopemfive time, there were significant differences at T3, T4 and T5 time point bwtween the two groups (P 〈 0.05). There were no significant differences in inlraoperative blood loss and intraoperative reinfusion ofautologous blood. Two patients in group A received allogeneic blood, while seven cases in group B, with significant differences (P 〈Z 0.05). Drainage and blood Wansfusion 24 hours after operation were not significantly different between the two groups (P 〈 0.05). Conclusion It is worth to generalize plasma separation combined with intraoperative autologous blood recovery technology, for it doesn' t increase the amount of blood loss and affect platelet function; moreover, it can replenish affect platelet function very well.
