摘要
目的探讨优质护理服务对护士付出-获得不平衡状况的影响,为更好地发展优质护理和促进护士群体健康提供参考。方法采用付出-获得不平衡(ERI)量表,对荆州市两所三级甲等医院348名护士实施优质护理服务前后的付出-获得不平衡状况进行问卷调查。结果优质护理服务前后护士ERI比值分别为1.38±0.47、1.27±0.34;ERI比例分别为84.48%、81.03%,差异无统计学意义(P>0.05);超负荷比例分别为72.99%、60.63%,差异有统计学意义(P<0.01)。在付出、获得及超负荷3个维度干预前后之间差异均有统计学意义(P<0.05或P<0.01)。结论优质护理服务改善了护士的"付出"状况,也增加了护士的"获得",减轻了护士的超负荷工作状态,但未能改变护士"付出-获得不平衡"状态。医院管理者应采取相应对策改善护士的高付出、低回报状况,促进优质护理持续、高质量发展。
Objective To investigate the impact of High-Quality Care Project on nurses' perceived effort-reward imbalance (ERI),and to provide reference for promoting high quality care and nurses' health. Methods Totally 348 nurses from two tertiary first-class hospitals were investigated with the Effort-Reward Imbalance Scale before and after the implementation of High-Quality Care Project. Results The ERI ratio was 1.38±0.47 and 1.27±0.34 before and after the implementation of High-Quality Care Project,respectively. There was no significant difference on the incidence of effort-reward imbalance before and after the implementation of High-Quality Care Project (84.48% vs. 81.03%,P〉0.05). The incidence of overcommitment decreased from 72.99% to 60.63%(P〈0.01). There were significant differences on the factor scores of effort, reward and overcommitment before and after the implementation of High-Quality Care Project (P〈0.01 or P〈0.05), Conclusion High- Quality Care Project can relieve nurses' perceived efforts and increase their rewards,reduce their overcommitment,but can't change nurses' perceived effort-reward imbalance. The hospital administrators should take actions to improve nurses' ERI condition and promote the sustainable implementation of high quality care.
