摘要
目的通过TSP-PCR技术,检测本地区皮肤疣患者(寻常疣、跖疣和扁平疣)感染HPV的型别,探讨皮肤疣自体植入疗效与HPV型别的关系。方法收集皮肤疣患者149例,通过皮肤疣自体植入治疗,同时取其疣体组织进行HPV基因型别检测。结果完成随访102例患者中HPV检出率高达99.02%(101/102),所设计的型别均有检出。其中多种HPV型别检出率较单一型别多,为58.42%。皮肤疣自体植入疗效表明,寻常疣、跖疣和扁平疣治疗有效率无统计学差异(χ2=0.225,P>0.05)。HPV单一和多种型别感染的治疗有效率差别无统计学意义(χ2=1.422,P>0.05)。<18岁组的治疗有效率明显高于>35岁组;病程<1年组有效率明显高于病程>3年组。结论皮肤疣自体植入治疗对单一及多种HPV型别感染均有效,对<18岁病程<3年的患者疗效较好,对>35岁病程>3年的患者疗效较差,这对于临床早期治疗具有现实指导价值。
Objective To detect the HPV genotype of local patients with skin warts, including verruca vulgaris, plantar warts and fiat warts, by using the type specificity primer PCR method, and to discuss the relationship between the curative effects of auto - verruca implantation and the HPV genotype. Methods Collect 149 patients with skin warts from the dermatological department outpatient of Zunyi Medical College affiliated hospital. Take the warts tissue for auto - verruca implantation treatment. Meanwhile, extracting genomic DNA samples from the skin warts, define and analysis the HPV gene se- quences and genotype, and discuss the relationship between the curative effects of auto - verruca im- plantation and the HPV genotype. Results Complete the follow - up of 102 patients, it is found that the total detection rate of HPV is 99.02% . And all the type designed was detected out. The most type that detected was multi - infections and the infection rate is 58.42%. The effective rates of auto - verruca implantation to treat verruca vulgaris, plantar warts and flat warts have no statistical significances of the differences（ X2 = 0. 225 ,P 〉 0.05 ）. The effective rate between mono -infection and multi -infections have no statistical significances of the differences（ x2 = 1. 422 ,P 〉 0.05 ）. The effective rate of patients no more than 18 years old which is obviously higher than that of the patients more than 35 years; the ef- fective rate of patients whose course of disease is more than 3 years which is obviously lower than that of the patients whose course of disease is less than 1 year. Conclusion Auto - verruca implantation is ef-fective not only for the mono - infection of HPV type of skin warts, but also for the HPV multi - infec- tions. The curative effect of auto - verruca implantation is better for the patients whose age is under 18 and whose course of disease is less than 3 years. However, the curative effect was worse of the patients whose age is above 35 and whose course of disease is more than 3 years. It has realistic significance of early treatment for skin warts patients.
