最大自主通气时隔肌肌电图的变化

摘要 本文对健康组和COPD组各11例受试者,在肺量仪上做1min量大自主通气(MVV),并分别在MVV之前、之后立即、2min和5min时作膈肌肌电图频谱分析,发现:(1)健康组和COPD组1min MVV前15s和后15s的通气量均无显著差异(P>0.05);(2)健康组MVV后立即的中心频率(Fc)较MVV之前显著降低(p<0.05),MVV之后2min、5min则无显著差别。高频/低频比值(H/L)MVV前后均无显著差别;(3)COPD组MVV后立即的Fc和H/L均有显著降低(P<0.05)。以上提示膈肌肌电图可能是检测隔肌疲劳较敏感的指标。 One minute maximal voluntary ventilation (MVV) was performed by 11 normal subjects and 11 patients of COPD group on a spirometer. Before, immediately after, and 2 minutes and 5 minutes after MVV, the diaphragmatic electromyogram of the power spectrum of the eximinees were recorded and analyzed.It was found there was no significant difference between the ventilation volume of the initial 15 seconds and the final 15 seconds of the 1 minute MVV in both the normal and COPD group (P>0.05) . In the normal control, the central frequecy (Fc) of the electromyogram before MVV was significantly higher than that immediately after MVV (P<0.05) ; in the 2nd and 5th minute after MVV, Fc of the normal returned to normal and showed no difference from that before MVV; and there was no significant difference between the high frequecy/ low frequency (H/L) before and after MVV. In COPD group, Fc immediately after MVV and H/L decreased significantly (P<0.05) .The findings suggest that electromyogram might be a sensitive index to determine the degree of fatigue of the diaphragm muscle.
