摘要
概述了目前国内外常用的两种糖尿病病人管理模式———自我管理(self-management)模式和全程管理(case management)模式的概念、实施方法和效果评价方式,并在此基础上探讨了既适合我国医疗卫生发展现状又能保证高质量服务和病人治疗效果的社区糖尿病病人管理模式。
This article summarized the concept,implementation methods and effect assessment of self-management model and the case management model that were common management models for diabetes patients in abroad and at home.And it probed into a new management model for diabetes patients in the community that was suitable for the development status of China's medical and health but also ensurea high-quality services and patients'outcomes
