摘要
目的探讨老年糖尿病患者静脉留置针穿刺局部感染的相关因素及处理措施,为降低穿刺局部感染率,减轻患者痛苦、提高患者生命质量提供依据。方法分析2008年1月-2010年12月268例年龄67～80岁老年糖尿病住院患者行静脉留置针后的穿刺局部感染情况。结果 268例行静脉留置针的老年糖尿病患者发生穿刺局部感染的有15例,感染率为5.60%;>70岁的老年糖尿病患者静脉留置针穿刺局部的感染率为9.52%,明显高于<70岁患者的3.07%,差异有统计学意义(P<0.05);静脉留置针留置时间>4d,穿刺局部感染的发生率高,达10.98%;行下肢静脉留置针的糖尿病患者穿刺局部感染的发生率为11.67%,明显高于行上肢静脉穿刺患者的3.85%,差异有统计学意义(P<0.05)。结论年龄>70岁、静脉留置针留置时间长是老年糖尿病患者感染的相关因素;选择正确的血管及穿刺部位、严格无菌操作、加强对患者血糖的控制等,是降低老年糖尿病患者静脉留置针穿刺局部感染的有效护理措施。
OBJECTIVE To study the related factors and measures for puncture local infections of vein needle retention in senile patients with diabetes mellitus so as to reduce the rate of puncture local infection of vein needle retention and the pain of the patients and improve the life quality of the senile patients with diabetes mellitus.METHODS A total of 268 senile patients with diabetes mellitus aged between 67 and 80 years old,who underwent the vein needle retention in the hospital from Jan 2008 to Dec 2010,were enrolled in the hospital,then the incidence of puncture local infections of vein needle retention was analyzed.RESULTS The puncture local infections of vein needle retention occurred in 15 of 268 senile patients with diabetes mellitus with the incidence rate of 5.60%.The puncture local infection rate of the senile diabetes mellitus patients aged more than 70 years old was 9.52%,significantly higher than 3.07%of the patients aged less than 70 years old,the difference was statistically significant（P0.05）.The longer the time of vein needle retention,the higher the incidence of the puncture local infection（10.98%,and the puncture local infection rate of senile diabetes mellitus patients with the lower limbs as the puncture local was 11.67%,significantly higher than 3.85% of the patients with upper extremity venous puncture,the difference was significant（P0.05）.CONCLUSIONThe more than 70 years of age and long time of vein needle retention are the related factors for the puncture local infections in the elderly patients with diabetes.The nursing measures for reducing the rate of puncture local infection of vein needle retention in senile patients with diabetes mellitus are choosing the correct blood vessels and the puncture locaion,strictly keeping the aseptic operation specifications,improving the proficiency of operation technology,paying close attention to the patients′condition,controlling the blood glucose of the patients,and promoting the good hygienic habits.
