金黄色葡萄球菌试管法血浆凝固酶试验影响因素的探讨 被引量:2

Study of the Factors Influencing Direct Tube Coagulase Test for Identification of Staphylococcus aureus

摘要 目的探究金黄色葡萄球菌试管法血浆凝固酶试验的影响因素。方法按《全国临床检验操作规程》方法操作,比较菌悬液的浓度、菌悬液介质、健康人血浆含抗凝剂种类以及血浆稀释度对试管法血浆凝固酶试验结果的影响。结果当菌液的浓度分别为2、3、4 MCF和>4 MCF时,含不同抗凝剂的人血浆凝块形成时间不一,只有当菌液的浓度>4 MCF时,且该菌悬液与稀释度不超过1:4的血浆混合时,这些血浆都凝固的最短时间为3.5 h;用生理盐水或去离子水配制的菌悬液与含同种抗凝剂血浆的凝固效果无差异。结论在金葡菌血浆凝固酶试验过程中,菌悬液浓度宜>4MCF;菌悬液介质可选择无菌去离子水或生理盐水;当血浆稀释度≤1:4时,可在3.5 h内观察结果;选择含肝素钠、枸橼酸钠和氟化钠抗凝剂的健康人血浆均可用于血浆凝固酶试验。 Objective To explore the factors affecting Staphylococcus aureus in the tube coagulase test.Methods According to the National Clinical Laboratory Operation Rules,we compared the effect of bacteria suspension concentration,bacteria suspension medium,the types of healthy plasma containing anticoagulants,and degrees of plasma dilution on plasma coagulase test results of the test tube method.Results When the bacterial liquid concentrations were 2MCF,3MCF,4MCF and 4MCF,the human plasma clots containing different anticoagulants were formed at different times.Only when the bacterial liquid concentration was 4MCF and the bacterial suspension mixing and dilution of the plasma was less than 1:4,the shortest time for the plasma to be frozen was 3.5hours.No difference was found in coagulation effect between bacterial suspension prepared with normal saline or deionized water and plasma containing the same anticoagulant.Conclusions The concentration of bacterial suspension should be more than 4 MCF in plasma coagulase test for detection of Staphylococcus aureus.Sterile deionized water or normal saline can be a good choice for bacterial suspension medium.When the degree of plasma dilution is ≤1:4,the detection result can be observed within 3.5 hours.Healthy plasma containing anticoagulants with heparin sodium,sodium citrate and sodium fluoride can be used in plasma coagulase test.
