透明质酸钠治疗后膝骨性关节炎滑膜病理改变 被引量:9

摘要 目的探讨透明质酸钠治疗后膝骨性关节炎(OA)滑膜病理的改变情况。方法将60例后膝OA患者按随机数字表法分为2组:观察组30例给予关节腔内注射液透明质酸钠治疗,对照组30例未给予任何治疗。治疗前后2组均进行膝关节镜检查,比较2组滑膜病理改变情况。结果治疗前2组骨性关节炎滑膜增生肥厚,毛细血管增生,淋巴细胞浸润和纤维化明显;治疗后观察组滑膜增生及淋巴细胞浸润显著改善。治疗后观察组患者Lequence指数评分明显低于对照组,2组比较差异具有统计学意义(P<0.05)。观察组治疗过程中未见肝功能、肾功能、血常规等生化项目的异常,无明显消化系统、神经系统及过敏症状发生。结论透明质酸钠治疗后膝骨性关节炎滑膜病理改善明显,安全性高,值得应用。 Objective To observe the pathological changes in synovium after sodium hyaluronate treatment of knee osteoarthritis(OA). Methods Sixty patients with OA were randomly divided into two groups, with 30 patients in each group. The observation group was treated with intra-articular injection of sodium hyaluronate. The control group received no treatment. Knee arthroscopy was performed before and after treatment to observe the pathological changes in synovium. Results Before treatment, patients with OA had synovial and capillary hyperplasia and lymphocytic infiltration and fibrosis. After treatment, synovial hyperplasia and lymphocytic infiltration were significantly improved in observation group. Compared with control group, I_equence indexes were obviously decreased by treatment with sodium hyaluronate (P〈0.05). No abnormalities in liver and kidney function and routine blood indexes were found and no digestive and nervous system lesions and allergy symptoms occurred in observation group. Conclusion Sodium hyaluronate is an effective and safe treatment for improving synovial lesions after knee OA.The effect of knee osteoarthritis synovium after sodium hyaluronate treatment is obvious and the side-effect is few, so they are worth being used.
