摘要
目的探讨乙型肝炎病毒感染孕妇血清白介素-2(IL-2)、白介素-4(IL-4)、白介素-6(IL-6)和干扰素-γ(IFN-γ)和与宫内感染的相关性。方法选择2009年2月~2012年11月收治的乙型肝炎病毒感染孕妇58例为研究组,根据有无宫内感染分为宫内感染组30例和宫内无感染组28例,选取同期30例健康产检孕妇为对照组,比较3组孕妇血清及新生儿脐血IL-2、IL-4、IL-6和IFN-γ的水平变化情况。结果新生儿宫内感染的发生率随孕妇HBV-DNA含量的增加而增高,差异有统计学意义(χ2=10.158,P<0.05)。宫内感染组孕妇血清IL-2、IFN-γ[(20.38±2.55)pg/mL,(20.77±3.02)pg/mL]低于对照组[(40.03±4.11)pg/mL,(32.84±4.11)pg/mL]和宫内无感染组[(40.03±4.11)pg/mL,(32.84±4.11)pg/mL],而IL-4、IL-6[(62.81±4.75)pg/mL,(110.22±7.18)pg/mL]则高于对照组[(31.25±3.88)pg/mL,(70.61±5.53)pg/mL],差异有统计学意义(P<0.05)上述指标宫内无感染组与对照组比较,差异无统计学意义(P>0.05)。3组新生儿脐血中IL-2、IL-4、IL-6和IFN-γ水平比较,差异无统计学意义(P>0.05)。对照组、宫内无感染组、宫内感染组孕妇血清IL-4和IL-6水平呈正相关(P<0.05),IL-2和IFN-γ水平呈正相关(P<0.05);血清IL-2和IL-6呈负相关(P<0.05),IL-2和IL-4呈负相关(P<0.05);IL-4和IFN-γ呈负相关(P<0.05);IL-6和IFN-γ呈负相关(P<0.05)。结论乙型肝炎病毒感染孕妇血清IL-4、IL-6、IL-2、IFN-γ水平升高,新生儿宫内感染的发生率随孕妇HBV-DNA含量的增加而增高。
Objective To explore the correlation of the serum IL-2 and IL-4,IL-6 and IFN-γ of intrauterine infection in pregnant women infected with hepatitis B virus.Methods 58 cases of pregnant women infected with hepatitis B virus from February 2009 to November 2012 were selected as research group and divided into intrauterine infection group(n = 30) and intrauterine uninfection group(n = 28) according to the presence of intrauterine infection,30 cases of healthy pregnant women were selected as control group at the same time;the level changes of blood serum and neonatal umbilical cord blood IL-2 and IL-4,IL-6 and IFN-γ were compared among three groups of pregnant women.Results The incidence of neonatal intrauterine infection were found increased with the increase of pregnant women HBV-DNA content,the differences were statistically significant(χ2 = 10.158,P 0.05).The serum IL-2,IFN-γ in intrauterine infection group [(20.38 ±2.55) pg/mL,(20.77 ±3.02) pg/mL] were lower than those in the control group [(40.03±4.11) pg/mL,(32.84 ±4.11) pg/mL] and intrauterine uninfection group [(40.03 ±4.11) pg/mL,(32.84 ±4.11) pg/ mL],but the IL-4,IL-6 [(62.81±4.75) pg/mL,(110.22±7.18) pg/mL] were higher than those of control group [(31.25 ± 3.88) pg/mL,(70.61±5.53) pg/mL],the differences were statistically significant(P 0.05);the above indicators in intrauterine infection group compared with control group,there was no statistically significant difference(P 0.05).The level of IL-2,IL-4,IL-6 and IFN-γ in neonatal umbilical cord blood were compared among 3 group,there was no statistically significant difference(P 0.05).The levels of IL-4 and IL-6 levels in pregnant women serum were found had positive correlation(P 0.05),IL-2 and IFN-γ were found had positive correlation(P 0.05);IL-2 and IL-6 were found had negative correlation(P 0.05),IL-2 and IL-4 were found had negative correlation(P 0.05);IL-4 and IFN-γ were found had negative correlation(P 0.05);IL-6 and IFN-γ were found had negative correlation(P 0.05).Conclusion Serum levels of IL-4,IL-6,IL-2,IFN-γ in pregnant women infected with hepatitis B virus have a rise,the incidence of neonatal intrauterine infection increases with the HBV-DNA of pregnant women.
