摘要
目的通过收集老年组(≥60岁)及中青年组(18～59岁)肺结核患者X线胸片及CT报告影像表现特点,分析老年组和中青年组影像特征是否存在差异。方法收集秦皇岛市第一医院,秦皇岛市中医医院,秦皇岛市传染病院肺结核住院确诊患者病历,分成老年组159例和青中年组255例。设计调查表,调查X线胸片及CT报告影像表现特点。结果 2组影像学表现:老年组结核病灶分布广泛,多分布中下肺野,大片实变影内多可见空洞,占24.53%;易合并感染,空洞内可见气液平面(10.06%);新旧病变同时存在,多见胸膜肥厚,好发结节影、结核球影及一侧一致性高密度影。而青中年组影像学表现相对单一,病变多分布两上肺叶,多以云絮状阴影为主(40.78%)及胸腔积液(26.27%)。2组纵膈淋巴结肿大及条索状、星状阴影及钙化阴影差别无统计学意义(P>0.05)。结论老年人肺结核病变影像表现为两肺大范围受累,多征象同时存在,表现复杂多样,易与其他疾病混淆。中青年人患病两肺受累范围较局限,征象表现单一。
Objective Through collecting imaging performance characteristics of Chest X-ray and CT reported of elderly（≥60 years） and young（18 ～ 59） patients with pulmonary tuberculosis,to analyze if there were differences of these imaging features in the elderly group and the young group.Methods Inpatient medical records of patients diagnosed with tuberculosis were collected from three hospitals（Qinhuangdao First Hospital,Qinhuangdao Hospital of Traditional Chinese Medicine and Qinhuangdao Infectious Hospital） including 159 elderly and 255 young patients.Imaging performance characteristics of Chest X-ray and CT of these medical records were investigated by questionnaire.Results The imaging in two groups showed：in elderly group there was tuberculosis widely distributed,mostly distributed in the lower lung fields and large opacities more visible within the cavity,accounting for 24.53%.Tuberculosis was easy to infection,voids visible liquid surface representing（10.06%） ;in younger group imaging has relatively homogeneous distribution of two upper lobe lesions,mostly cloud-based floc shadow（40.78%） and pleural effusion（26.27%）.There was no difference between two groups on mediastinal lymph nodes and cords,Stellate shadows and calcification shadows（P 〈0.05）.Conclusion Elderly people suffering from tuberculosis lesions have large-scale involvement of the lungs in imaging performance and more signs.Young people suffering from tuberculosis lesions have the more limited range of sick lungs and performance of a single signs.
