3种麻醉方式在小儿腹腔镜手术中的应用比较 被引量:18

摘要 目的比较七氟烷吸入、七氟烷与瑞芬太尼静吸复合麻醉及异丙酚与瑞芬太尼全静脉麻醉对短时腹腔镜手术患儿血流动力学、苏醒质量及围术期不良反应的影响。方法腹腔镜疝修补术或腹腔镜阑尾切除术的美国标准协会Ⅰ~Ⅱ级、2~8岁患儿90例,随机分为3组:S组采用七氟烷吸入诱导及维持麻醉;SP组采用异丙酚、瑞芬太尼静脉诱导,七氟烷、瑞芬太尼静吸复合维持麻醉;P组以异丙酚复合瑞芬太尼诱导及维持麻醉。3组患儿诱导成功后,均给予肌松剂气管插管。结果 S组诱导时间高于其他两组(P均<0.01)。气管插管、拔管、唤名睁眼和意识恢复时间3组无统计学差异(P均>0.05)。诱导及插管过程中,S组发生心动过速、高血压高于其他两组(P均<0.01),且术中心率均较基础值增快(P均<0.01)。3组苏醒期躁动、术后24 h恶心呕吐,组间有统计学差异(P均<0.01)。结论与七氟烷吸入麻醉相比,七氟烷与瑞芬太尼静吸复合麻醉及异丙酚与瑞芬太尼全静脉麻醉,术中血流动力学更加平稳,苏醒期躁动、术后24 h恶心呕吐发生率较低,更适用于小儿短时腹腔镜手术。
