摘要
目的探讨汉防已甲素在青光眼滤过性手术中抗瘢痕化的作用。方法将21只健康大耳白兔,随机平均分为三组:汉防已甲素组(tetrandrine,TET)、空白对照组、丝裂霉素C组(mnomycin C,MMC),均行常规小梁切除术,术中分别在巩膜瓣下放置浸有0.2%TET、0.002%MMC的棉片5 min,然后弃去棉片,用生理盐水冲洗干净,分别在术后第7、30、60 d检查眼压、滤过泡、角膜、眼底,并分别取滤过口附近组织4mm×2 mm,送电镜及病理学检查。结果 TET组眼压多控制理想,较对照组差异有统计学意义,多数能形成功能性滤过泡,无任何并发症,病理未见结缔组织胶原化。电镜下,汉防已甲素组见成纤维细胞胞体收缩,间隙增宽,线粒体较少,纤维细胞异染色质,合成蛋白质功能不活跃。结论汉防已甲素组在抗青光眼滤过术中的抗瘢痕化作用优于MMC,是当前较理想的抗代谢抗增殖的药物,值得推广应用。
Objective To explore the anti - scarring effect of tetrandrine in rabbit models with glaucoma filtering surgery. Methods Twenty - one healthy rabbits were randomly divided into three groups: tetrandrin(TET) group, blank control group and mitomycin C(MMC) group. Glaucoma filtering surgery was conducted. 0.2 % TET and 0. 002 % MMC cotton sheets were respectively placed under the scleral flap for 5 minutes, and then were discarded. The eyes were rinsed with saline. The in- traccular pressure, bleb, cornea and eyeground were checked on the 7th, 30th and 60th day after the surgery. Tissues near the filter with a size of 4 mm ~ 2 mm were collected for electron microscopy and histopathological examination. Results As compared with the control group, intraocular pressure was decreased perpetually in TET group and most of the models could form a functional bleb without any complication. There was no connective tissue collagen in pathology. Electron microscope observation showed that the fibroblasts shrank and the cell gap was widened. The protein synthesis in the fibroblasts in the TET group were inhibited. Conclusions The anti - scarring effect of tetrandrine is superior to that of MMC in the anti - glaucoma filtration surgery. It can be widely applied as the ideal anti - metabolism and anti - proliferation drug.
