摘要
目的观察炎调方治疗脓毒症急性呼吸窘迫征(热炽营血证)对患者外周血白细胞介素-2(IL-2)、白细胞介素-6(IL-6)、白介素-8(IL-8)和肿瘤坏死因子-α(TNF-α)的调控作用及临床疗效。方法将患者随机分为炎调方组22例和对照组23例,对照组予西医常规治疗,炎调方组在西医常规治疗的基础上加用炎调方,观察两组患者治疗前后外周血细胞因子水平、胸部X线或CT检查、动脉血气分析SaO2、氧合指数、使用机械通气的治疗时间和病死率等,评价两组中西医临床疗效。结果经治疗,两组外周血细胞因子IL-2、IL-6、IL-8和TNF-α水平均有明显下降,炎调方组低于对照组;两组SaO2、氧合指数水平均有明显提高,炎调方组均高于对照组;炎调方组存活患者机械通气治疗时间短于对照组;中西医临床疗效均有优于对照组的趋势。以上差异均有统计学意义(P<0.05或0.01)。结论炎调方通过下调外周血细胞因子水平而减轻炎症反应及对肺组织的持续损伤,治疗脓毒症急性呼吸窘迫综合征(热炽营血证)有确切的临床疗效。
Objective：To investigate and verify the effect of Yantiao Decoction on the level of serum cytokines,including interleukin-2（IL-2）,interleukin-6（IL-6）,interleukin-8（IL-8） and tumor necrosis factor-α（TNF-α） and the clinical efficacy.Methods：Patients were randomly divided into two groups：control group in which 23 subjects were treated with just western medicine and treatment group in which 22 cases were treated with western medicine and Yantiao Decoction with a course of 14 days.The levels of cytokines were determined before and after treatment,meanwhile,radiological changes,arterial blood gas,duration of mechanical ventilation and mortalities were monitored,and clinical efficacy was observed.Results：The serum levels of IL-2,IL-6,IL-8,TNF-α were significantly decreased after treatment in both groups（P0.01） and the post-treatment levels in the treatment group were significantly lighter than the post-treatment levels of control group（P0.05）.The levels of SaO2 and the indexes of oxygenation were significantly increased after treatment in both groups（P0.01） and the posttreatment levels in the treatment group were significantly higher than the post-treatment levels of control group（P0.05）.Compared to the control group,duration of mechanical ventilation in survival patients was significantly shorter in the treatment group（P0.01）.Meanwhile,the traditional Chinese medicine and western medicine clinical efficacy of treatment group was significantly better than which of the control group.Conclusion：Yantiao Prescription can significantly decrease the levels of serum cytokines and lighten constant lung injury made by inflammatory response,and clinical efficacy in the treatment of sepsis complicated ARDS /syndrome of heat blaze in ying-blood with Yantiao Prescription is actually comfirmed.
