单侧椎弓根螺钉联合对侧椎板关节突螺钉固定微创治疗腰椎退行性疾病 被引量:7

Unilateral pedicle screws combined with contralateral translaminar facet fixation in minimally invasive treatment of lumbar degenerative disease

摘要 目的观察单侧椎弓根螺钉联合对侧椎板关节突螺钉固定微创治疗腰椎退行性疾病的疗效。方法共35例患者,术中单侧显露、减压,同侧椎弓根螺钉固定,MED引导下对侧进行椎板关节突螺钉固定。观察手术时间、术中出血量和术后引流量,术后评价椎板关节突螺钉位置,观察术前、术后椎间隙高度,观察术后内固定有无松动或断裂、椎间融合器有无移位。结果手术切口小,术中出血少,术后切口引流少。切口无感染、皮肤坏死,无脑脊液漏、神经损伤。与术前相比,术后椎间隙高度改善,随访未出现明显丢失,融合率高。随访中椎弓根螺钉与椎板关节突螺钉无松动、断裂,椎间融合器无移位。结论单侧椎弓根螺钉联合对侧椎板关节突螺钉固定方法微创、稳定性好、融合率高。 Objective To observe the curative effect of unilateral pedicle screws combined with contralateral translaminar facet fixation in minimally invasive treatment of lumbar degenerative disease.Methods 35 patients received unilateral exposure,decompression,the same side fixed by unilateral pedicle screw,the opposite side fixed by contralateral translaminar facet under guidance of MED.The operation time,blood loss during operation and the postoperative drainage volume were observed.The position of contralateral facet screw after operation,the change of intervertebral space height,the loosening or fracture of internal fixation,and the cage displacement were evaluated.Results The operation had a small incision,less bleeding,less postoperative drainage volume,and no incision infection,skin necrosis,cerebrospinal fluid leakage and nerve injury occured.Compared with the preoperation,postoperative intervertebral space height was improved obviously.After the operation,there was no significant loss,and had high rate of fusion.During the follow-up,pedicle screws and translaminar facet screw did not loosen and fracture,and the cage had no displacement.Conclusion Unilateral pedicle screws combined with contralateral translaminar facet screw has advantages of minimally invasive,stability,high fusion rate.
