摘要
目的通过荧光定量PCR(FQ-PCR)方法检测血清HBV-DNA阳性产妇产后脐带血及乳汁HBV-DNA载量,探讨胎儿宫内感染HBV及乳汁携带HBV与产妇HBV-DNA载量的相关性。方法 2010年1月至2012年11月,选取在产科住院分娩的HBV-DNA阳性产妇149例,采用FQ-PCR方法检测产妇产前血清、产后脐带血和乳汁HBV-DNA载量。结果①选择有HBV感染史产妇304例中149例HBV-DNA阳性,阳性率为49.01%(149/304);②149例HBV-DNA阳性产妇胎儿宫内感染为42.95%;HBV-DNA阳性孕妇乳汁中HBV-DNA阳性率(71.14%)显著高于脐血(42.95%)(P<0.01);HBV-DNA≥1×105组脐带血、初乳及满月乳中HBV-DNA阳性率(61.29%、82.80%、100.00%)显著高于1×103≤HBV-DNA<1×105组(12.50%、51.79%、75.00%)(P<0.01);满月乳中HBV-DNA阳性率(90.60%)显著高于初乳(71.14%)(P<0.01)。结论 HBV-DNA阳性孕妇胎儿HBV的宫内感染率及乳汁中携带HBV的阳性率高,与孕妇血清HBV-DNA载量正相关,阻断HBV的宫内感染及减少乳汁携带HBV者,必须有效的降低孕妇血中的HBV-DNA载量水平。
Objective Through the FQ-polymerase chain reaction (PCR) to the measurement of serum HBV-DNA positive maternal postpartum cord blood and milk HBV-DNA capacity, this paper discusses the fetus intrauterine infection HBV and milk carry HBV and maternal HBV-DNA and appropriateness of relevance.Methods anuary 2010 to November, select in the maternity hospital obstetric HBV-DNA positive maternal 149 patients, the FQ-PCR detection antenatal serum, postpartum umbilical cord blood and milk HBV-DNA capacity.Results Choose to have HBV infection maternal history of 304 patients in 149 cases of HBV-DNA positive,positive rate was 49.01%(149/304). 149 cases of HBV-DNA positive maternal fetal intrauterine infection was 42.95 %.HBV-DNA positive women breast milk HBV-DNA positive rate (71.14 %) was statistically higher than that in umbilical cord blood (42.95 %) (P〈0.01).HBV-DNA was lx105 group of umbilical cord blood and eolostrum milk and full moon of HBV-DNA positive rate (61.29 %, 82.80 %, 100.00 %) is significantly higher than lx103 or less HBV-DNA 〈lx105 group (12.50 %, 51.79 %, 75.00 %)(P〈0.01). he full moon in milk HBV-DNA positive rate (90.60 %) was statistically higher than that in colostrum (71.14 %)(P〈0.01).Conclusions HBV-DNA positive women fetal HBV infection rates and the palace in the milk of carrying HBV positive rate is high,pregnant women with serum HBV-DNA capacity increases in infection rates and milk carry HBV are high,the block of HBV intrauterine infection and reduce milk carry HBV person, must effectively reduce blood of pregnant women in the HBV-DNA capacity level.
