摘要
【目的】探讨子痫前期患者血清脑利钠肽(BNP)水平的变化及与子痫前期的关系。【方法】收集本院产科门诊检查和住院分娩的子痫前期115例,其中重度57例(中孕期27例,晚孕期30例),轻度58例(中孕期28例,晚孕期30例),并收集同期正常妊娠者90例(中、晚孕期各45例),回顾性分析所有受检孕妇血清BNP水平。【结果】①重度子痫前期血清BNP值为(225.31±43.7)pg/mL,显著高于轻度(28.31±3。8)Pg/mL及正常妊娠(17.5±3.2)pg/mL,且差异有显著性(均P〈0.05);轻度子痫前期血清BNP值与正常妊娠比较差异无显著性(P〉0.05)。②随着孕周的增加,正常妊娠血清BNP值在整个孕期比较稳定,中孕期为(17.8±1.2)pg/mL,晚孕期为(17.6±1.5)pg/mL,两者相比差异无显著性(P〉0.05);轻度子痫前期中孕期为(27.8±1.2)pg/mL,晚孕期为(28.6±1.5)pg/mL,两者相比差异无显著性(P〉0.05);重度中孕期为(178.8±1.2)pg/mL,晚孕期为(265.6±1.5)pg/mL,血清BNP值随孕周明显升高,且差异有显著性(P〈0.05)。【结论】①妊娠期患者血清BNP水平的变化与患者病情的严重程度相关;②妊娠期患者血清BNP水平明显增高可能与子痫前期的发生发展有关,BNP可作为重度子痫前期的早期预测指标。
[Objective]To explore the change of serum B-type natriuretic peptide(BNP) in patients with pre-ecl^mpsia pregnancy and its correlation with pre-eclampsia. [Methods] Totally 115 cases of pre-eclampsia examined in clinic service and delivered in our hospital were collected. Among them, 57 patients(27 cases of middle pregnancy and 30 cases of late prenancy) were severe and 58 patients (28 cases of middle pregnancy and 30 cases of late pregnancy) were mild. Concurrent 90 normal pregnant women(45 cases of middle pregnancy and 45 cases of late pregnancy) were also collected. Serum BNP in pregnant women was analyzed retrospec- tively. [Results]Serum BNP in severe pre-eclampsia (225.31 ± 43.7) pg/mL] was markedly higher than that in mild pre-eclampsia[(28.31 ± 3.8) pg/mL and patients with normal pregnancy[( 17.5 ± 3.2) pg/mL], and there was significant difference(all (P 〈0.05). There was no significant difference in serum BNP between mild pre-eclampsia and normal pregnancy( P 〈0.05). With the increasing of pregnancy time, serum BNP in nor- mal pregnancy was stable in all pregnancy, and those in middle pregnancy and late pregnancy were (17.8 ±1.2)pg/mL and (17.6±1.5)pg/mL respectively, but there was no significant difference( P 〉0.05). Serum BNP of mild pre-eclampsia in middle pregnancy and late pregnancy were (27.8 ± 1.2)pg/mL and (28.6 ± 1.5) pg/mL respectively, but there was no significant difference( P 〉0.05). Serum BNP of severe pre-eclampsia in middle pregnancy and late pregnancy were (178.8 ± 1.2)pg/mL and (265.6 ± 1.5)pg/mL respectively, and increased with pregnancy time, and there was significant difference(P〈0.05). [Conclusion]The change of se- rum BNP in patients during pregnancy is related to the disease severity of patients. The increasing of serum BNP in patients during pregnancy may be related to the occurrence and development of pre-eclampsia. Serum BNP can be used as the early prediction index of severe pre-eclampsia.
