摘要
目的探讨阶梯法治疗分泌性中耳炎的临床疗效。方法选取2011年11月—2012年11月本院收治的分泌性中耳炎患者239例,随机分为治疗组(n=120)和手术组(n=119)。治疗组给予抗生素治疗、鼓膜穿刺、鼓膜置管术及病灶周边组织切除术,手术组给予鼓膜置管和病灶周边组织切除手术。治疗结束后评价2组患者的治疗效果。结果治疗组总有效率为97.50%,手术组总有效率为96.64%,2组治疗总有效率差异无统计学意义(P>0.05);治疗组中经药物治疗治愈者34例,鼓膜穿刺治愈者39例,鼓膜置管治愈者23例,腺样体刮除术者8例(有6例同时行扁桃体切除术),治愈率为86.67%。结论阶梯法治疗分泌性中耳炎安全有效,能够实现与手术治疗同样的效果,值得临床推广应用。
Objective To study the therapeutic effect of staircase method in the treatment of secretory otitis media.Methods A total of 239 secretory otitis media patients who were admitted to our hospital from November 2011 to November 2012 were randomly divided into the treatment group（n=120） operation group（n=119）.The treatment group received antibiotic ＋ puncture of tympanum,grommet insertion,resection of lesions surrounding tissue,while the operation group received tympanostomy tube insertion and perilesional tissue resection operation.After treatment,the treatment effects of the two groups were evaluated.Results The overall efficacy rate of the treatment group was 97.50%,compared with 96.64% in the operation group.The difference of the two groups was not statistically significant（P〈0.05）.In the treatment group,34 patients were cured after drug treatment,39 patients were cured with tympanic membrane puncture,23 patients were cured with tympanostomy tube,and 8 patients received curettage of adenoids（6 patients underwent tonsillectomy）.The total cure rate was 86.67%.Conclusion Staircase method in the treatment of secretory otitis media is safe and effective,which can achieve the same effect with surgery treatment.Thus it is worthy of wide clinical applications.
