摘要
目的探讨多层螺旋计算机断层扫描(MSCT)联合核磁共振成像(MRI)在腕关节损伤诊断中的应用价值。方法将150例腕关节损伤患者随机分为MSCT组48例,MRI组49例及联合组53例,分别采用MSCT、MRI及MSCT+MRI检查进行诊断。比较3组患者骨折处、关节脱位例数、移位碎骨折片数量、隐匿性骨折处、腕关节处其他损伤的发现率,以及损伤范围的差异。结果 MSCT组发现骨折处、关节脱位例数、移位碎骨折片数量均显著多于MRI组;而MRI组发现隐匿性骨折处、软骨损伤及骨挫伤处均显著多于MSCT组,肌腱情况及皮下组织损伤范围的确诊情况也优于MSCT组,而联合组上述指标的确诊情况均显著优于其他2组。结论将MSCT与MRI联合应用于腕关节损伤的诊断,可有效提高骨折、关节脱位、隐匿性骨折、软骨损伤、骨挫伤及移位碎骨折片的检出率,并有助于确诊肌腱及皮下组织损伤的范围,为临床治疗方案的制订提供更为科学的诊断依据,可在临床加以推广应用。
Objective To explore the application value of muhilayer spiral CT (MSCT) combined with MRI in the diagnosis of wrist joint injury. Methods One hundred and fifty patients with wrist joint injury were divided into MSCT group ( n = 48), MRI group ( n = 49) and combination group ( n = 53). The three groups received MSCT, MRI, MSCT + MRI examination respectively. The numbers of fracture, joint dislocation, dislocated pieces, occult fractures, other injuries of wrist joints and injury range of the three groups were compared. Results The numbers of fractures, joint dislocation, dislocated pieces in the MSCT group were obviously bigger than those in the MRI group, while the numbers of occult fracture, cartilage injuries and other injuries of wrist joints in the MRI group were obviously bigger than those in the MSCT group, and the diagnosis condition of injury range of tendons and subcutaneous tissue was also better than the MSCT group. The diagnosis condition of above - mentioned indexes in the combination group were superior than the other two groups. Conclusion MSCT combined with MRI can be used for diagnosis of joint injury, can effectively improve relevance ratio of fracture, joint dislocation, occult fractures, cartilage injury, bone contusion, dislocated pieces, and also help identify the injury range of tendon and the subcutaneous tissue. It can provide more scientific diagnosis basis for the establishment of clinical treatment, and is worthy of popularization and application.
