摘要
目的探讨高效价冷凝集素对血常规血型鉴定和交叉配血的影响及消除方法。方法通过临床病例分析、红细胞ABO血型鉴定、血清抗体筛选、抗体鉴定、冷凝集素效价测定等血型血清学检测来查找配血困难原因。结果高效价冷凝集素常造成一些常规检验项目失真,最常见的影响就是对血常规,血型鉴定,尤其是对交叉配血的干扰。与自身红细胞发生明显凝集,使血型鉴定出现失误,并干扰交叉配血,因错误输血而导致自身免疫性红细胞破坏,引发严重的临床症状。结论高效价冷凝集素干扰交叉配血时,应选择有价值的鉴别试验,如复核血型、自身对照试验,抗人球蛋白试验、聚凝胺法交叉配血试验等进行多方面排除、鉴定,以确保临床用血的安全有效性。
Objective To investigate the effect of high titer cold agglutinin on clinical cross-matching test and its eliminate measures. Methods The result of cross-matching test trouble that high titer cold agglutinins caused was searched by medical record analysis, ABO blood group identification, serum antibody screening, antibody identification, fold agglutinin titration, etc. Results The reason of disturbing routine blood, blood group identification and cross-matching blood test, the presence of high-fiter cold agglutinins usually cause routine inspection items misfit by causing obvious agglutination with its own red blood cells, and make mistakes to blood type identification, or interfere with cross-matching blood test, sometimes destruct autoimmune red blood cell caused by transfusion errors, even trigger severe clinical symptoms. Conclusion When high-titer cold agglutinins interfering with cross-matching blood test, valuable identification tests should be thought so as to make sure with safety and efficacy of blood clinical using, such as depleted and identified by countercheck blood type, autoantibody absorption test, antiglobulin test of coombs, polybrene cross-matching blood test, irradiation teat.
