摘要
目的评价伊曲康唑治疗肝衰竭合并侵袭性肺部真菌感染患者的疗效和安全性。方法回顾性分析伊曲康唑治疗肝衰竭合并侵袭性肺部真菌感染患者的有效性和安全性。治疗方法:静脉注射伊曲康唑0.2g静脉滴注q12h1天或者2天后改为伊曲康唑0.2g静脉滴注qd,疗程2—4周。结果共有16例患者符合人选标准和排除标准纳入本研究,男女比例为3:1。其中4例由白色念珠菌感染所造成,4例为热带念珠菌感染,8例为曲霉感染。临床结果:4例肝衰竭合并白色念珠菌感染患者,其中3例治愈出院,1例死于肝衰竭。4例肝衰竭合并热带假丝酵母感染患者,其中2例治愈出院,1例死于肺部感染,1例死于肝衰竭。8例肝衰竭合并肺曲菌病感染患者中,5例感染控制,其中2例治愈出院,2例接受肝移植,1例死于肝衰竭。3例肺部感染加重,死于呼吸衰竭。结论伊曲康唑治疗肝衰竭合并侵袭性肺部真菌感染安全有效,可以作为治疗肝衰竭合并侵袭性肺部真菌感染患者的选择。
Objective To evaluate the efficacy and safety of itraconazole in the treatment of invasive fungal infection among pa- tients with hepatic failure. Methods A retrospective study was conducted to assess the efficacy and safety of itraconazole in treating inva- sive fungal infection in patients with hepatic failure. Treatment protocol : intravenous itraconazole 0.2 g ql2h on Day 1 and Day 2, fol- lowed by 0.2 g iv qd for 2-4 weeks. Results A total of 16 patients were enrolled with a male to female ratio of 3 1, including four pa- tients with infection caused by Candida albicans, four with Candida tropicalis infection, and eight with Aspergillus infection. Clinical out- comes : of the four patients who suffered from hepatic failure and invasive Candida albicans infection, three patients were cured of fungal infection and discharged while one died from hepatic failure. Of the four patients who had Candida tropicalis infection, improvement was observed in two patients who were then safely discharged; one patients died from pulmonary infection and one died from hepatic failure. A- mong the eight patients who had pulmonary aspergillosis, infection was controlled in five patients, of whom, two patient was discharged and two received liver transplant later, one died from hepatic failure. Three patients had exacerbation of pulmonary infection and died from respiratory failure. Conclusion Itraconazole is a safe and effective treatment option for invasive fungal infection among patients with he- patic failure.
