摘要
目的:探讨在接受冠状动脉造影(冠造脸查的人群中吸烟与冠造特点的关系。方法:人选1997—09至2001—07在我院行冠造的患者4174例,分为吸烟组(n=1936)和无吸烟组(n=2238例),研究两组患者中吸烟史和吸烟量与是否存在冠状动脉病变及严重程度是否相关。结果:吸烟组与无吸烟组比较,男性、年龄小于40岁以及有心肌梗死病史的患者比例较高,冠造结果阳性率也较高(82.8%VS66.2%,P〈0.001)。经多因素校正后,吸烟是冠造阳性的独立预测因素(OR=1.756,95%CI=1.54~2.01,P〈0.01)。在3084例冠造阳性的患者中,单支血管病变患者中大量吸烟、少中量吸烟和不吸烟的比例分别为39.3%、11.6%和49.0%,而多支血管病变患者中其比例分别为37.7%、14.7%和47.6%,二者比较差异无统计学意义(P〉0.05)。多因素Logistic回归分析提示,年龄≥60岁、女性、高血压、2型糖尿病是冠状动脉多支病变的独立预测因素。
Objective: To explore the correlation between cigarette smoking with smoking amount and the occurrence of coronary artery disease (CAD) with the severity of coronary lesions by coronary angiography (CAG). Methods: A total of 4174 patients who received CAG examination in our hospital from September 1997 to July 2001 were retrospectively studied. All patients were divided into 2 groups, Smoking group, n=1936 and Non-smoking group, n=2238, The relationship between smoking with smoking amount and CAD occurrence with the severity of coronary lesions was studied. Results: Compared with Non-smoking group, Smoking group showed younger age, higher ratios of male gender, angina pectoris, myocardial infarction (MI) and positive CAG findings, all P〈0.001. With adjusted multi factors, smoking increased the risk of positive CAG findings (OR=1.756, 95%CI=1.54-2.01, P〈0.001. There 3084 patients had positive CAG results and among them, for patients with 1-vessel disease, the percentage of heavy smoking, moderate smoking and non-smoking were 39.3%, 11.6% and 49.0% respectively; for patients with multi vessel disease, the percentage for heavy smoking, moderate smoking and non-smoking were 37.7%, 14.7% and 47.6 respectively; P〉0.05. Multi logistic regression analysis indicated that elder age (≥ 60 years), female gender, hypertension and diabetes were the independent risk factors for patients suffering from coronary multi-vessel disease. Conclusion: In patients who received CAG examination, the positive rate was more in smokers, while the smoking amount was not related to the severity of coronary lesions.
