摘要
目的:探讨针对家庭结构的心理护理对抑郁症患者及其照顾者的影响。方法:选择124例住院治疗的抑郁症患者,随机分为观察组和对照组各62例。对照组采用抑郁症常规护理,观察组采用针对家庭结构的心理护理;分别在干预前和干预后第12、24周,依据汉密尔顿抑郁量表(HAMD)和汉密尔顿焦虑量表(HAMA)对2组患者进行评分,依据症状自评量表(SCL-90)和家庭功能评定量表(FAD)对其照护者进行评分。结果:在入组第12、24周,2组的HAMD、HAMA评分及抑郁、焦虑、人际关系敏感因子分、SCL-90总分、行为控制、角色沟通和FAD总分差异均有统计学意义(P<0.01);SCL-90总分与控制因子分呈正相关关系(P<0.05),SCL-90总分、焦虑、HAMA因子分与家庭总功能均呈正相关关系(P<0.05),躯体化与沟通分呈正相关关系(P<0.05)。结论:针对家庭结构的心理护理可以增强抑郁症患者疗效,减轻其照顾者抑郁、焦虑情绪,改善家庭功能,值得临床推广。
Objective：To explore the influence of psychological care involving family members on patients with depression and their caregivers. Methods：One hundred and twenty-four hospitalized patients with depression were randomly divided into observation group and control group received. The observation group received the psychological care involving the family members and the control group received the routine care. All the patients were tested by Hamilton Depression Scale（HAMD） and Hamilton Anxiety Scale（ HAMA）, and their caregivers were tested by Symptom Checklist（SCL-90） and Family Functional Assessment Scale（FAD） before and the 12th and 24th week after the intervention. Results： In the 12th and 24th week, the differences in the HAMD, HAMA, depression, anxiety, interpersonal sensitivity factor points, SCL-90 total score, behavior control, communication and FAD score between the observation group and the control group were statistically significant（P 〈 0.01 ） ;There was a positive correlation between the SCL-90 total score and the control factors （P 〈 0.05） ;the SCL-90 total score, anxiety and HAMA factor points had a positive correlation with the total household function（P 〈 0.05） ;The somatization was positively correlated with the communication （P 〈 0.05 ）. Conclusions：Psychological care involving family members can greatly enhance the curative effect on patients with depression and relieve their caregivers＇ depression and anxiety, improving the family function. It is worthy of popularization.
