准分子激光原位角膜磨镶术矫正近视的并发症 被引量:2

Complications of excimer laser in situ keratomileusis for correction of myopia

摘要 目的分析准分子激光原位角膜磨镶术(LASIK)治疗近视的术中、术后并发症,探讨其发生原因及防治措施。方法642例(1273眼)近视按术前等效球镜度分为3组,均行LASIK手术且术后随访12个月,分析术中、术后并发症及手术前术后裸眼视力分布情况。结果术中并发症主要有角膜瓣形成不全、游离角膜瓣、角膜瓣偏位、薄角膜瓣、角膜层间异物残留、球结膜下出血、及角膜血管翳出血;术后并发症有眩光及夜视力下降、干眼、角膜瓣褶皱、角膜瓣下浑浊、上皮植入、弥漫性层间角膜炎、过矫、欠矫、屈光回退及调节紊乱等;术后12个月时93.73%达到或超过手术前最佳矫正视力。结论LASIK术矫正近视安全有效,但术中和术后仍存在并发症,提高手术技巧和完善手术设计可有效减少并发症的发生。 Objective To analyze operation process and complications of laser in situ keratomileusis (LASIK), in the correction of myopia, and to assess cause of cases and control measures. Methods 1273 eyes of 642 cases with myopia were divided into three groups according to spherical equivalent. All cases received LASIK, following up 12 months postoperatively. Operation process and during and postoperative complications and distribution of uncorrected vision were analyzed. Results Complications during operation included mainly incomplete cap, free flap, corneal flap deflection, thin corneal flap, foreign body in the interface, subconjunctival hemorrhage, corneal pannus hemorrhage and so on. Postoperative complications included glare and decreased visual acuity at night, dry eye, corneal flap fold, clouding under corneal flap, epithelial implantation, diffuse lamellar keratitis ( DLK), overeorrection, undereorrection, refractive regression, accommodative disorder and so on. 93.73% of patients met or exceeded best corrected visual acuity than that before surgery. Conclusion LASIK in the correction of myopia is safe and effective, but there are still some complications during operation and postoperation. Improved surgical skills and advanced surgi- cal design can reduce complications effectively.
