摘要
目的:介绍局部皮瓣在修复面部创面的临床经验。方法:在遵循美容亚单位组成的原则基础上,根据创面大小、形状、部位及创面周围皮肤情况,选择相应局部皮瓣修复缺损,并使辅助切口顺应面部分区自然轮廓线(鼻唇沟、鼻翼沟、颌鼻缝、睑缘、唇红缘、发际等)或皮肤皱纹,尽可能隐藏切口线。结果:本组局部皮瓣191例,仅2例皮瓣尖端发生坏死,其余皮瓣均完全成活,切口Ⅰ期愈合,形态较满意,随访6个月~2年,2例局部切口瘢痕增生,但无功能受限及邻近器官移位。结论:应用局部皮瓣修复面部缺损可使皮瓣的色泽、质地、厚度、毛发生长情况与受区匹配,面部轮廓影响小,功能与外观两者可以兼顾,能够获得较好的美容修复效果。
Objective： To introduce the clinical experience of local flaps in repairing facial defect. Methods： Based on the aesthetic subunit principle and acording to the size, shape, location and the condition of the defect, we selected homologous local flaps to cover the defect and incisions were made along the lines of the natural contours of the facial subregion lines （nasolabial fold, sulcus alaris, maxillonasal suture, palpebral margin, vermilion border, hair line, etc） or skin wrinkles to conceal incisions. Results： Among 191 flaps, only 2 flaps necrotized at distal end and the others recovered completely, inci- sions healed by first intention and achieved satisfied effect. Patients were followed up for 6 months^2 years, 2 incisions ap- peared obvious scar, but did not affect the function and have no apparatus displacement. Conclusion： Using local flaps to re- pair the facial defect can make flaps match the recipient site with contour, texture, thickness and hair grow condition, have little affect to the facial contour and acquire good effect in both aspect and function.
